高龄老人膝关节骨关节炎行全膝关节表面置换术的疗效观察

目的：观察全膝关节表面置换治疗高龄老人膝关节骨关节炎的短期临床疗效。方法回顾性分析2006年8月至2011年8月在本组研究接受全膝关节表面置换术的高龄老年膝关节骨关节炎患者58例（60膝），其中男18例（20膝），女40例（40膝）；年龄75～82岁，平均77.6岁。采用HSS评分标准评估手术前后膝关节功能，观察有无并发症发生。结果所有患者术后均获随访，随访时间2.5～5.0年，平均3.5年。患者HSS平均评分由术前（38.00±8.12）分升高至术后最终随访时（90.00±7.94）分，术后优良率达92.9％。6例出现患侧下肢深静脉血栓，经抗凝及对症处理后痊愈。5例（5膝）切口愈合障碍。结论全膝关节表面置换治疗高龄老年膝关节炎短期临床疗效良好。

Clinical observation of elderly knee osteoarthritis undergoing total knee arthroplasty

Objective To evaluate the clinical effects of the total knee arthroplasty （ TKA） on the treatment for knee osteoarthritis in the elderly patients .Methods From August 2006 to August 2011, 58 patients （60 knees） with arthritis underwent total knee arthroplasty in our hospital , including 18 males （20 knees） and 40 females （40 knees）.The average age was 77.6 years （range, 75-82 years）.The knee functions were evaluated by the HSS scores .The complications were also observed .Results All the patients were followed up for 3.5 years in average （ range, 1.5 -4.5）.The HSS scores increased from （ 36.00 ±7.43 ） preoperatively to （ 89.00 ±6.95 ） postoperatively .The excellent and good rate was 89.5%.Seven cases developed deep venous thrombosis and were cured by anticoagulant therapy and other symptomatic treatments .Five cases （ 5 knees ） had the wound healing disorder .Conclusion Total knee arthroplasty may achieve good short-term clinical outcomes in treating knee osteoarthritis in the elderly patients.
