自固定轻量补片在腹腔镜下腹股沟疝修补中的应用

目的分析自固定轻量补片在腹腔镜下腹膜前腹股沟疝修补术中的临床疗效。方法回顾性分析2007年8月至2012年4月，天津市泰达医院收治的腹股沟疝患者100例。随机分为二组，观察组50例，应用自固定轻量补片行腹腔镜下腹膜前腹股沟疝修补术；对照组50例，采用传统补片（普通聚丙烯补片）行腹腔镜下腹膜前腹股沟疝修补术。观察二组患者的手术时间、治疗费用、恢复时间、并发症、复发率、术后短期疼痛情况。结果二组患者手术时间、治疗费用差异有统计学意义（t=2．481、2．223，P=0．021、0．023）。二组患者恢复时间差异无统计学意义（t=1．732、P=0．123），并发症发生率差异无统计学意义（X2=1．435，P=0．246）。二组患者术后1dVAS分数比较差异无统计学意义（t=1．703，P=0．136）。观察组术后7d，1、6个月VAS评分与对照组相比差异有统计学意义（t=2．356、2．513、2．869，P=0．037、0．029、0．012）。二组均无复发病例。结论对腹股沟疝患者采用腹腔镜下经腹膜前自固定轻量补片术，有利于降低手术难度，节约治疗费用，减轻患者的疼痛，值得在临床上进行推广。

Self-fixed light-weight patch in laparoscopic preperitoneai repair of inguinal hernia

Objective To analyze the effect of self-fixed light-weight patch in laparoscopic preperitoneal repair of inguinal hernia. Methods A total of 100 cases with inguinal hernia were enrolled and retrospectively analyzed in Tianjin Taida Hospital from August 2007 to April 2012. All the patients were randomized into two groups. In the observation group, 50 patients were applied with self-fixed light-weight patch and treated with laparoscopic preperitoneal inguinal hernia repair. While 50 cases were applied with traditional patch and abdominal laparoscopic preperitoneal inguinal hernia repair. The operation time, treatment costs, recovery time, complications, rates of recurrence, and postoperative pain were assessed. Results There were statistically significant differences in the operation time and treatment costs （ t = 2. 481, 2. 223, P = 0. 021,0. 023 ）. No significant differences in recovery time （ t = 1. 732, P = 0. 123 ） or complication rates was observed between the two groups X2 = 1. 435, P = 0. 246）. One day after operation, VAS scores had no statistical significant differences （t = 1. 703, P = 0. 136）. Seven days after operation, VAS had statistical significant differences in the Month 1 compared to Month 6 （t = 2. 356,2. 513,2. 869, P = 0. 037,0. 029,0. 012）. The two groups had no recurrence. Conclusions Self-fixed light-weight patch was applied in the patients with inguinal hernia, which is beneficial for reducing the difficulties of operation, pain of patients, and saving the treatment costs.
