多项指标检测系统性红斑狼疮的临床意义

目的 探讨可提取核抗原(ENA)多肽抗体谱、抗双链DNA(dsDNA)抗体、免疫球蛋白和补体对系统性红斑狼疮(SLE)的诊断价值.方法 测定84例SLE患者及90例健康人血清自身抗体、补体和免疫球蛋白;测定45例经4个月治疗后SLE患者的自身抗体、补体和免疫球蛋白水平.结果 84例SLE患者的抗dsDNA、抗Sm-D1、抗Ro/SS-A60、抗Ro/SS-A52、抗SSB、抗U1-SnRNP、抗Histon、抗Po 和抗Nucle检测阳性率分别为73.8%、84.8%、83.3%、61.9%、45.2%、42.9%、33.3%、30.9%和9.5%;SLE 患者免疫球蛋白值明显高于健康对照组,补体C3 、C4 明显低于健康对照组,差异均有统计学意义(P<0.05).45例SLE患者治疗4个月后血清抗dsDNA、抗Sm-D1、抗Ro/SS-A60、抗Ro/SS-A52、抗SSB、抗U1-SnRNP、抗Histon、抗Po 和抗Nucle检测阳性率分别为55.6%、55.6%、88.9%、55.6%、55.6%、22.2%、22.2%、11.1%和33.3%.结论 自身抗体、免疫球蛋白及补体联合检测有助于SLE的诊断分型和预后评估.

Clinical significance of multiple detection indexes for diagnosing systemic lupus erythematosus

Objective To investigate the value of polypeptide antibodies spectrum of extractable nuclear antigens（ENA）,anti double-stranded DNA（anti-dsDNA） antibodies,immunoglobulin and complement in the diagnosis of systemic lupus erythematosus（SLE）.Methods Autoantibodies immunoglobulin and complement in 84 patients with SLE and 90 healthy people were detected;autoantibodies immunoglobulin and complement in 45 patients with SLE treated four months were detected.Results In 84 patients with SLE,the positive rates of anti-dsDNA,anti-Sm-D1,anti Ro/SS-A60,anti-Ro/SS-A52,anti-SSB,anti-U1-SnRNP,anti-Histon,anti-Po and anti-Nucle test were 73.8%,84.8%,83.3%,61.9%,45.2%,42.9%,33.3%,30.9% and 9.5% respectively.Immunoglobulin in the SLE group was significantly higher than that of the control group,complement C3,C4 were significantly lower than those of the control group,showing statistical difference（P0.05）.The positive rates of serum anti-dsDNA,anti-Sm-D1,anti Ro/SS-A60,anti-Ro/SS-A52,anti-SSB,anti-U1-SnRNP,anti-Histon,anti-Po and anti-Nucle detection in 45 SLE patients treated for 4 months were 55.6%,55.6%,88.9%,55.6%,55.6%,22.2%,22.2%,11.1% and 33.3% respectively.Conclusion The combination detection of autoantibody,immunoglobulin and complement is beneficial to diagnostic classification and prognostic evaluation of SLE patients.
