摘    要:目的:验证急性脑出血患者的脑血流(CBF)不受血压(BP)降低影响的假设。方法:150例自发性脑出血发病后24h内收缩压〉150mmHg的患者被随机分到静脉降压组,目标是收缩压〈150mmHg(n=78)或〈180mmHg(n=72)。用基线收缩压平衡治疗组:(182±20)mmHg(〈150mmHg组)与(184±25)mmHg(〈180mmHg组,P=0.60),血肿量:(25.6±30.8)与(26.9±25.2)mL(P=0.66)。随机后2h,〈150mmHg组(140±19)mmHg vs.(1624-12)mmHg,P〈0.001)]的平均收缩压明显降低,均行CT灌注成像。结果:主要终点:〈150mmHg组(0.86±0.12)血肿周围组织相对CBF未明显低于〈180mmHg组(0.89±0.09,P=0.19;绝对差异0.03;95%CI-0.018~0.078)。〈150mmHg(R=0.00005;95%CI-0.001~0.001)或〈180mmHg组(R=0.000,95%CI-0.001~0.001),BP变化程度和血肿周围相对CBF间无关系。结论:中等量脑出血发病后快速降低血压不会降低血肿周围脑血流,表明快速降低血压不会使脑出血患者陷入脑缺血状态。

Abstract:Objective: To explore the effect of blood pressure on cerebral bloOd llOW OI patients with acute cerebral hemorrhage. Methods: A total of 150 cases with cerebral hemorrhage who had systolic pressure )150 mmHg were selected as veinous depressurization group, including 78 cases with blood pres- sure 〈150 mmHg, 72 cases 〈180 mmHg. Group with base line treatment:(182±20)mmHg (〈150 mmHggroup)] and (184±25)mmHg (〈180 mmHg group, P= 0. 60) Hematoma volume: (25.6± 30.8)and(26.9±25.2) mL (P =0.66) . 2 h after grouping, systolic pressure was significantly reduced in 〈150 mmHg group (140±19) mmHg vs. (162〈12) mmHg, P〈0. 001) ]. All had CT perfusion weigh-ted imaging. Results. Relative CBF in hematoma peripheral tissue of 〈150 mmHg group (0.86±0.12)was not significantly lower than that of (180 mmHg group (0.89〈0.09,P=0.19〈 difference 0.03〈 95%CI -0.018-0.078). 〈150 mmHggroup (R =0.00005〈 95%CI-0.001-0. 001)and(180 mmHg group (R =0. 000,95 %CI -0. 001-0. 001). The change in blood pressure had no association of cerebral blood flow. Conclusions. Rapid depressurization after moderate cerebral hemorrhage can't decrease peripheral hemato- ma cerebral blood flow. It indicates rapid depressurization would not induce cerebral ischemia.
