胰十二指肠切除术后早期肠内营养对感染性并发症的影响

目的 探讨胰十二指肠切除术(PD)术后早期肠内营养(EN)对术后感染性并发症的影响.方法 收集我院2011年1月至2011年10月间行PD的患者共46例,回顾性分析术后早期EN和肠外营养(PN)对术后感染并发症的影响.结果 两组间比较,术后9d EN组内毒素和(1-3) -β-D葡聚糖明显低于PN组(P<(0.05);EN组术后腹腔感染和腹腔外感染总发生少于PN组(P<0.05);EN组术后吻合口并发症发生少于PN组(P<0.05).结论 PD术后早期行EN可以有效降低术后腹腔内感染、腹腔外感染、真菌感染和吻合口并发症发生率,改善预后.

The effect of early enteral nutration on infected complications after pancreaticoduodenectomy

Objective To investigate the effect of early enteral nutration(EN) working on infected complications in patients undergoing pancreatoduodenectomy(PD).Methods Retrospect 46 patients’ clinical information who underwent PD with early EN or parenteral nutrition(PN) post-operation from Jan.to Oct.,2011,to analyze the effect of infected complications.Results The endotoxin and(1,3){beta}-D-glucan levels of patients in EN group was lower than those in PN group and had remarkable differences on the 9th day after operation(P <0.05).The incidence of intra-abdominal infection and extra-abdominal infection in EN group was lower than those in PN group significantly(P <0.05).Stoma complications after surgery in EN group were less than PN group(P <0.05).Conclusion Early postoperative EN reduces clinical infection rates,including intra-abdominal infection,extra-abdominal infection,invasive fungal infection,and complications in patients after PD.
