实时三维超声成像评价超声微波凝固治疗范围的实验研究

摘    要:目的:探讨实时三维超声成像(RT-3DE)技术在评价离体肝脏行微波凝固(PMCT)的范围及价值。方法:离体犬肝脏12个(共80个凝固点),进行PMCT并实时三维观察及实时全容量三维成像(FV-3DE),结合全容积分析软件,脱机计算出凝固区的范围及体积。结果:RT-3DE即刻显示肝脏局部图像内任一结构的三向正交切面和空间毗邻关系,FV-3DE具有快速显示肝脏整体凝固区体积的优点,与实际体积相比较无明显差异(P>0.05)。结论:RT-3DE技术可观察到微波凝固治疗过程中凝固区的变化、边缘、轮廓以及凝固区的体积,对介入凝固治疗有重要指导意义。

Abstract:Objective: To study the imaging characteristics and extent of real-time three-dimensional echocardiography(RT-3DE) during and after the percutaneous microwave coagulation therapy(PMCT). Methods: Twelve resected dog livers were treated by PMCT with different doses. At the same time, RT-3DE imaging was performed to study the local image changes including extent and echo. The volume of coagulation were assessed by RT-3DE system, with volumetric analysis software of RT-3DE system offline and compared to the water replace meal method. Results: Three orthogonal planes and spatial relationship of any liver tissue structure within images were instantly displayed with RT-3DE, no significant difference were found between the volume of coagulation by FV-3DE and water replace meal method(P>0.05). Conclusion: The spatial relationships of the coagulated extent of liver tissue were observed during the PMCT treatment. The edge outline and echo-change of destroyed tissue could be detected by 3D imaging during and after PMCT. Real-time 3D ultrasound is a new imaging method that may be effective in therapy-guidance during and after PMCT.
