来氟米特治疗难治性肾病综合征的研究

目的观察新型免疫抑制剂来氟米特治疗难治性肾病综合征的临床疗效。方法将41例难治性肾病综合征患者随机分为治疗组21例和对照组20例,治疗组采用中等量激素联合来氟米特治疗,对照组采用中等量激素联合环磷酰胺(CTX)冲击治疗,观察2组的疗效及不良反应,随访时间12个月。结果治疗组:完全缓解7例,部分缓解9例,无效5例,复发2例,有效率76%,复发率10%,无明显肝毒性、骨髓抑制等不良反应。对照组:完全缓解5例,部分缓解6例,无效9例,复发6例,有效率55%,复发率30%,有肝毒性及骨髓抑制等不良反应,2组有效率、复发率比较有显著性差异(P<0.05)。结论中等量激素联合来氟米特治疗难治性肾病综合征疗效显著,不良反应少,安全性好,患者易耐受。

Study of leflunomide on refractory nephrotic syndrome

Objective It is to observe the clinical curative effect of the new immune suppressive agent leflunomide on refractory nephrotic syndrome. Methods 41 patients with refractory nephrotic syndrome were randomly divided into two groups. 21 patients were in treatment group and treated with medium amount hormone combined with leflunomide. 20 patients were in control group and treated with medium amount hormone combined with CTX. Then the curative effect and untoward reaction of the two groups were observed. The follow up time was 12 months. Results In treatment group, seven cases completely relieve, nine cases partly relieve, five cases nullity and two cases relapse, and the effective rate was 76 %, the relapse rate was 10%, and there was no obvious hepatoxicity, bone marrow depression and so on untoward reactions. In contrast group, five cases completely relieve, six cases partly relieve, nine cases nullity and six cases relapse, and the effective rate was 55%, the relapse rate was 30%, and there were hepatoxicity, bone marrow depression and so on untoward reactions. There were significant different on effective rate and relapse rate between the two groups ( P <0.05). Conclusion Medium amount hormone combined with leflunomide on refractory nephrotic syndrome has significant curative effect and little untoward reaction. By also, it is safe and tolerable.
