下肢静脉曲张术后复发原因及手术方法探讨

目的探讨下肢静脉曲张术后复发原因及外科治疗方法。方法分析合肥市第二人民医院2003年1月-2008年12月收治的73例85条肢体下肢静脉曲张术后复发患者的临床资料,所有患者同时进行彩色多普勒超声检查和顺行性下肢静脉造影,以评估下肢静脉功能,并根据不同的复发原因选择相应的术式。结果复发的原因有：大隐静脉主干残留及大隐静脉剥脱不全15条,原发性深静脉瓣膜功能不全未纠正44例,交通支静脉功能不全21条,血栓形成后遗症5条。本组采用手术方式：大隐静脉高位结扎及剥脱术15例,股静脉缩窄术44例,内镜下交通支离断术21例,腘静脉肌襻成行术5例。同时施行腔内激光治疗和环行缝扎术。结论下肢静脉曲张术后复发的原因较为复杂,其主要原因是深静脉瓣膜功能不全。术前全面的检查和选择正确的手术方式是防止下肢静脉曲张术后复发的关键。

Cause and Treatment of the Recurrence of Varicose Veins after Varicotomy in Lower Extremity
