腹腔镜阑尾切除术术中阑尾系膜的不同处理方式

目的探讨腹腔镜阑尾切除术术中多种阑尾系膜处理方式的优缺点。方法 260例腹腔镜阑尾切除术使用6种方式处理阑尾系膜,其中钛夹、结扎夹夹闭法20例,PK刀、超声刀离断法18例,单极电凝法110例,双极电凝法13例,丝线结扎法96例,直线切割闭合器（EndoGIA）离断法3例。比较6种处理方式的疗效、手术时间及预后。结果 260例腹腔镜阑尾切除术术中分别使用不同的阑尾系膜处理方式,均顺利完成手术。结论在确保止血效果的前提下,应根据术者的经验和能力、设备条件及患者的经济承受能力选择适宜的方法。

The different treatment of mesenteriolum in laparoscopic appendectomy

Objective To explore the advantages and disadvantages of different treatment on mesenteriolum in laparoscopic appendectomies.Methods According to different treatment of mesenteriolum in laparoscopic appendectomy,260 cases performed with laparoscopic appendectomy were divided into 6 groups.Titanium clips and ligating clip clipping method 20 cases,plasma knife or ultrasound knife group 18 cases,monoplanar electrocoagulation group 110 cases,bipolar coagulation group 13 cases,silk suture ligation group 96 cases,EndoGIA group 3 cases.Compared the efficacy,operation time and prognosis of the six treatment methods.Results Two hundred and sixty cases laparoscopic appendectomies were completed successfully with different modus to mesoappendix.Conclusions Proper treating method should be chosen according to experience and ability of the surgeon,equipment condition of the hospital and financial capacity of the patient to ensure the hemostasis effect.
