| 摘 要： | 目的探讨低压灌注下经尿道前列腺四区分割法双极等离子体前列腺剜切术（PKRP）,治疗前列腺增生（BPH）的疗效和安全性。方法将经尿道前列腺镜像分割为四区,即中区（约占30%）、左右下区（约占40%）和上方区（约占30%）。依据患者年龄、前列腺体积、国际前列腺症状评分（IPSS）、生活质量评分（QOL）、尿流率、tPSA、全身状况、合并症、经济状况等多因素进行综合分析,采用低压灌注下双极等离子体剜切术治疗BPH患者198例,分别切除不同体积的前列腺增生组织,其中切除70%～100%者156例;50%～70%者33例;50%以下9例,随访103例,占52.8%,随访时间3～12个月。结果前列腺平均体积52ml,切除组织46～185g不等,平均手术时间65min。平均失血量140ml。术后留置尿管2～4d,住院时间3～9d。切除前列腺体积50%以上者,术后最大尿流率（Qmax）、IPSS、QOL、剩余尿（PVR）均有明显的改善。结论低压灌注经尿道前列腺＂四区分割法＂双极等离子体前列腺剜切术治疗BPH,切除50%以上体积前列腺组织即可达到较好的手术效果,安全性高,并发症少,恢复快。
