透明质酸钠辅助治疗肱骨远端粉碎性骨折术后疗效分析

目的探讨透明质酸钠治疗及预防肱骨远端粉碎性骨折双钛板内固定术后关节粘连,僵硬,创伤性关节炎的临床疗效。方法将2009年10月²012年12月期间开展肱骨远端粉碎性骨折双钛板内固定术患者随机分为对照组(创面及关节腔内未做任何处理)和观察组(创面涂抹及关节腔内注射医用透明质酸钠)。观察两组患者术后肿胀,疼痛,引流量,肘关节功能,术后创伤性关节炎的发生率。结果两组患者术后都没有出现切口感染,观察组患者也没有出现药物及异物排斥反应,术后肿胀,疼痛,引流量较对照组少,关节功能明显优于对照组,观察组术后创伤性关节炎的发生率低。结论透明质酸钠能有效减轻肱骨远端粉碎性骨折术后粘连的发生,避免关节僵硬而引起肘关节功能障碍。能保护关节软骨,润滑关节腔,抵抗感染,参与软骨修复,从而避免术后创伤性关节炎的发生。

ANALYSIS OF SODIUM HYALURONATE IN ADJUVANT THERAPY OF POSTOPERATIVE COMMINUTED DISTAL HUMERAL FRACTURE

Objective To investigate the clinical curative effect of sodium hyaluronate, which in the treatment and prevention of synarthrophysis, ankylosis and traumatic arthritis after the surgical operation of double titanium - plate inner fixation for eomminuted fractures of the distal humerus. Methods Randomly divided the patients into control group（ the wound and the joint cavity without any treatment） and observation group（ with wound smearing and intra- articular injection of sodium hyaluronate）, who accepted the surgical operation of double titanium -plate inner fixation for comminuted fractures of the distal humerus during the period from 2009 October to 2012 December. Re- suits The patients of （the）two groups did not have infection of incisional wound. Patients of the observation group did not have drug rejection and foreign body rejection, patients with postoperative swelling or ache were less than the control group＇s, their joint function significantly （ is ）better than the control group＇s. Also, the observation group had a low incidence of traumatic arthritis. Conclusion Sodium hyaluronate can effectively reduce the incidence of sy- narthrophysis after the surgical operation for eomminuted fractures of the distal humerus, avoid joint stiffness, whicn could lead to dysfunction of elbow joint. It can protect articular cartilage, lubricate the joint cavity, resist infection, repair the cartilage, （ and ） avoid the occurrence of postoperative traumatic arthritis.
