门静脉高压症脾切除术后持续性发热原因的探讨及处理

目的探讨门静脉高压症脾切除术后持续性发热的原因及处理方法。方法对1998年8月-2009年5月本院收治的肝硬化门静脉高压患者脾切除术后持续性发热患者18例（术后体温〉38．5℃，持续性发热2周以上者）临床资料进行回顾性分析。结果除2例原因不明外，术后发热均由各种并发症引起，以门、脾静脉血栓形成和胰尾损伤最常虬。持续性发热与肝功能分级显著相关（P〈0．01）。结论门脾静脉血栓形成及胰尾损伤是术后持续性发热的主要原因，防治门脾静脉血栓形成及改进手术方式可有效地减少术后持续性发热。

The causes and management after splenectomy persistent fever in patients with portal hypertension

Objective To investigate the cause and management of aflerleneetomy persistent fever after splenectomy in portal hypertensive patients. Methods From Augest 1998 to May 2009, 18 eases with pesslstent fever after splenectomy in portalhypertensive patients were analyzed retrospectively. Results Except for 2 patients with unknown cause, 16 of 196 patients with continuous fever were caused by complications induing portal or splenic vein thrombosis 7 cases, injury of pancreative tail 5 cases,thoracic hydropsy 2 cases and left sahodiaphragmatie hydropsy 2 cases. The lasting fever was related to the grade of liver function （P〈0.01）. Conclusions Portal, splenic vein thrombosis and injury of pancreas are the main causes of persistent fever after spleneetomy. Prevention and treatment of portal or splenic vein thrombosis and improving of operative procedures would help to reduce the rate of postoperative continuous fever.
