林格液、高渗盐水、高渗甘露醇在失血性休克初期急救中的应用效果比较

目的比较林格液、高渗盐水、高渗甘露醇在失血性休克初期急救中的应用效果。方法将失血性休克患者随机分成乳酸林格液组(A组)、乳酸林格液+高渗盐水组(B组)、乳酸林格液+高渗甘露醇组(C组),每组20例,比较各组的升血压及降心率效果。结果B、C两组给药后升血压及降心率效果明显优于A组;B组药效持续时间长于C组;C组出现3例甘露醇血管外渗致组织损伤。结论快速输注高渗盐水、高渗甘露醇在失血性休克初期急救时均有显著效果,高渗盐水药效持续时间更长,副作用少,为首选。

Application of Ringer solution, hypertonic saline solution and hypertonic mannitol solution in hemorrhagic shock patients in early treatment

Objective To compare the therapeutic effects of Ringer solution, hypertonic saline solution and hypertonic mannitol solution in hemorrhagic shock patients in early treatment. Methods Sixty patients with hemorrhagic shock were randomly and averagely divided into three groups: group A: lactate Ringer solution; group B: lactate Ringer solution and hypertonic saline solution; group C: lactate Ringer solution and hypertonic mannitol solution. The pressor and depressor bradycardia effects were compared. Results The effect of the pressor and depressor bradycardia in group B and C was better than that in group A; Lasting time of the drug in group B was longer than that in group C. Three patients occurred tissue damage caused by mannitol extravasation in group C. Conclusion In early treatment,infusing hypertonic saline and hypertonic mannitol solution rapidly in hemorrhagic shock patients have significant effect, but infusing hypertonic saline has the features of prolonged drug effect and lower side effect. Therapeutic method of group B is the best choice.
