骨髓增生异常综合征患者骨髓细胞免疫表型研究及意义

摘    要:为探讨骨髓增生异常综合征(MDS)各亚型的骨髓细胞免疫表型变化及临床意义,采用免疫酶标法检测30例MDS患各亚型36例次骨髓细胞抗原的表达,并进行临床追踪观察及检测。研究结果表明,髓系细胞抗原表达明显异常,早期髓系细胞抗原CD13,CD33阳性表达率明显增加,较成熟细胞抗原CD15阳性表达率减少;单核细胞抗原CD14及淋巴细胞抗原CD7,CD10无显异常,干/祖细胞抗原CD34在MDS的RAEB/RAEB-t组显增加,RA和RAS组则增加不明显;早期红系细胞及各增殖期细胞抗原CD71在MDS各亚型均明显高于对照组。结论说明MDS患骨髓细胞免疫表型存在多系异常,且对协助诊断及判断预后有一定意义。

Abstract:The phenotypes of the bone marrow cells in various subtypes of myelodysplastic symdromes (MDS) and its clinical implication were explored. The antigen expression of a panel of antigens expressed in marrow cells from 30 patients with subtypes of MDS were assayed by alkaline phosphatase anti alkaline phosphatase method. The results showed that the expression of myeloid antigens appeared abnormality, CD13 and CD33, found on granulocyte and macrophage precursors, increased, and CD15 decreased. There were no significant changes for monocytic antigen CD14 and lymphoid antigens CD7 and CD10. CD34 was increased in RAEB/RAEB t and was not increased in RA/RAS patients. CD71, expressed by erythroblast and proliferative cells, was higher in all subtypes of MDS than that in control group. It is suggested that the bone marrow cells from MDS patients showed abnormality of more than two series of immunophenotypes, detection of immunophenotype in MDS cells might be contributed to the diagnosis and predicting prognosis.
