| 摘 要： | 目的调查并分析2008—2012年河北医科大学第三医院儿童胫骨近端骨折的年龄、性别特征及结构分布情况。方法对2008年1月—2012年12月河北医科大学第三医院收治的新鲜儿童胫骨近端骨折患者资料进行回顾性分析。将患儿按年龄分为4个年龄段：<3岁、3~6岁、7~11岁、12~16岁。记录不同年龄段患儿的性别及骨折国际内固定研究学会/儿童长骨骨折( AO/PCCF)分型等数据,并进行统计学分析。结果5年间河北医科大学第三医院共诊治儿童骨折8987例,其中胫腓骨骨折1163例,胫骨近端骨折118例；胫骨近端骨折占儿童全身骨折的1.3%(118/8987),占儿童胫腓骨骨折的10.1%(118/1163)。118例胫骨近端骨折患儿中,男性80例(67.8%),女性38例(32.2%),男性多于女性。随患儿年龄增加,其骨折所占比例呈增高趋势。胫骨近端骨折男女高发年龄均为12~16岁,年龄段构成比52.5%(62/118)。骨折AO/PCCF分型中,41t-E型骨折78例(66.1%,78/118),其中41t-E/7型骨折最多,45例(38.1%,45/118)；41t-M 型40例(33.9%,40/118),其中41t-M/2型和41t-M/7型较多,各17例(14.4%,17/118)。不同年龄段41t-E/7型、41t-M/2型骨折分布差异有统计学意义(字2=16.657, Hc=32.702, P值均<0.05)。结论儿童胫骨近端骨折男性多于女性,好发年龄为12~16岁,高发骨折类型为41t-E型,尤其是41t-E/7型。儿童胫骨近端骨折的重点预防人群为大龄男性儿童。
