经阴道超声检查早期诊断宫颈功能不全的价值

目的：探讨经阴道超声检查早期诊断宫颈功能不全的临床应用价值。方法：采用经阴道超声对25例宫颈功能不全的孕妇于中孕时进行检查诊断,并与经腹超声检查结果进行对比分析,以评价经阴道超声检查在早期诊断宫颈功能不全中的价值。结果：经腹超声检查诊断符合17例,符合率达68%,误诊4例,占16%,漏诊4例,占16%;经阴道超声诊断宫颈功能不全符合23例,符合率可达92%,漏诊1例,占4%,误诊1例,占4%。23例宫颈功能不全孕妇中,13例行选择性环扎术,足月分娩9例,早产2例,流产2例;10例行保守治疗,足月分娩5例,早产3例,流产2例。结论：经阴道超声检查早期诊断宫颈功能不全是目前安全、直观、准确的检查方法,具有重要临床价值。

The value of transvaginal ultrasound in early diagnosis of cervical incompetence

Objective： To explore the clinical value of transvaginal ultrasound in early diagnosis of cervical incompetence. Methods： Twenty five cases of patients with cervical incompetence treated in our hospital received retrospective analysis and these pregnant women were diagnosed during pregnancy. The data of diagnosis were compared with that of transvaginal ultrasound to evaluate the value of transvaginal ultrasound in early diagnosis of cervical incompetence. Results： There were 17 consistent cases in transabdominal ultrasound diagnosis with the coincidence of 68% and 4 cases under misdiagnosis （16%）. Besides, 4 cases missed diagnosis （16%）. There were 23 consistent cases in tranvaginal ultrasound diagnosis with the coincidence of 92% and 1 case missed diagnosis （4%） that was similar to the number of misdiagnosis. 13 cases received selective cerclage in 25 patients with cervical incompetence. Of the 13 cases, 9 cases were presentede delivery and 2 cases miscarriage, 10 cases received expectant treatment including 5 cases with teiIn delivery, 3 cases with premature delivery and 2 cases with miscarriage. Conclusions： Transvaginal ultrasound is a safe, accurate and intuitive technology having significant clinical value in early diagnosis of cervical incompetence.
