脊柱外伤及手术后并发下肢深静脉血栓形成的预防

目的:探讨脊柱外伤及手术后急性下肢深静脉血栓形成(DVT)的预防方法。方法:回顾本院2000年1月~2006年3月资料完整的1094例脊柱外伤及手术病例,发生有症状的DVT 26例。其中2003年2月以前病例均未采取血栓预防措施(A组);2003年2月~2004年3月对于高危病例,采用以低分子肝素为主的预防措施(B组);2004年4月~2006年3采用维生素E、1,6-二磷酸果糖(FDP)及低分子肝素联合预防DVT(C组)。比较3组深静脉血栓形成的发生率。结果:A组病例570例,发生DVT22例;B组病例242例,发生DVT 3例。C组病例282例,发生DVT 1例。DVT发生率A组>B组>C组。未出现出血等并发症。结论:对于深静脉血栓高危患者,伤后或围手术期进行维生素E、FDP及低分子肝素联合预防措施效果满意。

Prevention of Deep Vein Thrombosis of Lower Extremity after Spine Trauma and Surgery

Objective To explore the preventive ways of acute deep vein thrombosis(DVT) of lower extremity after spine trauma and surgery.Methods Reviewed the clinical data of 1094 patients with spine trauma and surgery from Jan.2000 to Mar.2006,in which there were 26 patients with deep vein thrombosis of lower extremity patients before Feb 2003 did not take preventive methods(Group A);To the high dangerous patients from Feb 2003 to Mar 2004 undergone preventive thrombosis methods mainly by low molecular weight heparin(Group B);To the high dangerous patients from Apr.2004 to Mar.2006 undergone preventive thrombosis methods by Vitamin E and fructose 1,6-diphosphate(FDP) and low molecular weight heparin(Group C).Compared the rate of the three groups.Results DVT occurred in 22 cases of 570 cases in Group a;DVT occurred in 3 cases of 242 cases in Group b;DVT occurred in 1 case of 282 cases in Group c.Incidence of DVT in group A is highest,that of group B second to it and with that of group C is lowest.No haemorrhage complication.Conclusion For patients with high dangerous of thrombosis of lower extremity,prevention treatment with Vitamin E and fructose 1,6-diphosphate(FDP) and low molecular weight heparin is satisfied after spine trauma and perioperation.
