大肠腺瘤及大肠癌细胞动力学的免疫组织化学观察

摘    要:收集的活检标木包括:正常大肠粘膜5例、大肠腺瘤18例、大肠癌8例,应用体外组织培养技术掺入5-溴脱氧尿核甙(BrdUrd),并以Brd Urd单克隆抗体免疫组织化学方法检测具有分裂能力的阳性细胞,计算标记指数(LI),观察细胞增殖带范围。结果发现:大肠腺瘤及大肠癌的LI明显高于正常粘膜(P<0.001)°腺瘤伴不同程度异型增生的LI也不同,中、重度异型增生高于轻度异型增生腺瘤(P<0.05),而与腺癌无明显差异。正常粘膜增殖带局限于隐窝下半部,而腺瘤及大肠癌的具有分裂能力的细胞则散布整个肿瘤细胞区域。这一结果证实从异型增生发展到癌的组织发生过程。

Abstract:Biopsy specimens of normal mucosa (n=5), adenomas (n=18), adenocarcino-mas (n=8) of the large bowel were investigated by an immunohistochemical methodusing 5-bromodeoxyuridine (Brd Urd). The Labeling Index (LI) was significantlylower in normal mucosa than in adenomas and adenocarcinomas. The proliferativezone was confined to the lower two-third of the crypt in the normal mucosa, but itwere either present in the upper third or scattered along the crupt and in surfaceepithelium. LI of adenocarcinomas did not statistically differ from that of adenomaswith moderate and severe grade dysplasia. The results support the adenoma-carci-noma sequence.
