香丹、参麦注射液合低分子量肝素治疗急性脑梗塞临床观察

目的：观察香丹，参麦注射液配合低分子量肝素静脉滴注，治疗急性脑梗塞的临床疗泖及其毒副作用。方法：将100例经头颅CT确诊为急性脑梗塞的患者按1：1随机分成治疗组与对照组，治疗组给予香丹注射液及参麦注射液各20ml加入5％葡萄糖盐水或0．9％生理盐水250ml(合并糖尿病者）静滴，1日1次，同时给予低分子量肝素钙5000抗Xa腹壁皮下注射，每12h 1次，对照组给予低分子右旋糖酐500ml静滴，1日1次，两组病例均根据脑水肿的程度使用不同剂量的甘露醇脱水治疗，同时配合应用胞二磷胆碱，肌苷、ATP、辅酶A等营养神经及改善脑细胞代谢药物， 治疗3周。结果：治疗组基本治愈率50％，总有效率92％，对照组基本治愈率22％，总有效率78％。两组比较，P＜0．01。治疗中未见明显副作用，结论：中西医结合疗法治疗急性缺血性脑卒中可以明显提高疗效。

Therapeutic Efficacy Report of Acute Cerebral Infarction Cases Treated with Xiang-Dan and Shen-Mai Injection Combined with Low Molecular Heparin

Objective: To observe the clinical efficacy and side effects of integrated treatment of acute cerebral infarction cases with Xinng Dan and Shen Mai Injection combined with low molecular weight heparin.Methods:100 CT diagnosed acute cerebral infarction cases were divided into two groups: Treatment group treated with Xiang Dnn and Shen Mai Injection intravenously once a day (250 ml of 5% G.N.S+20 ml injection; or 250 ml of0.9% N.S +20 ml injection forthe diabetes cases), together with hepacarin 5000u for abdominnl suboutaneous, three times a day. The control group cases were treated with 500 mi of low molecular Dextran. Mannitol was given based on the cerebral edema conditions. Corenalin,inosiae, ATP and coaplin were given to improve the cerebral functions. Attention was paid to the maintaince of water and electrolyte balance, antibiotics, controlling the blood pressure, blood sugar level and nerve function restoration therapies. After two weeks' treament,the evaluation of the nervous function defect level was made according to the 1995 National conference for cerebral va,scalar diseases. Results: The primary curative rate was 50% with 20% total cure in the integrated group. The control had 22% primary curative rate, P <0.01. No side effects were observed in the treatment. Conclusion:Xiang Dan and Shen Mai injections were effective in treating acute cerebral infarction cases without side effects.
