药物性肝损害40例临床分析

目的提高对药物性肝损害的认识。方法对2003年4月～2007年7月住我院的40例药物性肝损害患者进行回顾性分析,对患者的一般资料、临床表现、实验室检查结果及涉及的药物进行总结。结果女性22例,男性18例。潜伏期多为1～8w。临床分型:肝细胞型24例(60.0%),胆汁淤积型5例(12.5%),混合型11例(27.5%),肝细胞型是最常见的临床类型。由西药引发肝损害27例(67.5%),中药9例(22.5%),多种药物(如中药 西药,成分不详的减肥药)4例(10.0%)。临床治愈34例(85.0%),好转4例(10.0%),死亡2例(5.0%)。结论多种药物均可引起药物性肝损害,应予以重视,临床上以肝细胞型为主,经积极治疗多数预后良好。

Clinical analysis of 40 cases with drug-induced liver disease

Objective To investigate the clinical characteristics of patients with drug-induced liver disease and drugs resulting in the liver injuries. To emphasize the importance of the prevention, diagnosis and therapy of the disease. Methods 40 cases of drug induced liver damage admitted to our hospital from 2003 to 2007. They were reviewed with respect to their drug history,clinical symptoms and signs,liver function test,serum viral markers and therapeutic effect. Results The number of female patients(55%)was more than the male patients(45%). The latent period varied form 1 to 8 weeks. Many drugs could induce liver disease. The main drugs involved were Western medicine in 67.5% and herbal medicine in 22.5%. The most frequent clinical type was the hepatocellular damage type(60%). Of all patients, 34 were curable(85%),4 were improved(10%), and 2 died(5%). Conclusion The various drugs may cause the drug-induced liver disease. The importance of drug-induced liver disease should be emphasized. Hepatocellular damage type is the most common type. The prognosis of most patients is optimistic by active treatment.
