内镜活组织印片刷片细胞学检查与活检联合使用诊断高位胃癌251例分析

本文分析8096例胃镜检查中发现的251例高位胃癌的临床和内镜检查资料。251例高位胃癌占同期胃癌总数的28.2％。40岁以上中老年男性居多,合并出血者占40.3％。内镜肉眼观察的诊断准确率(94.8％),明显高于X线钡餐透视的诊断正确率(37.9％)。绝大多数是进展期癌,早癌仅3例(1.2％)。印片,刷片细胞学检查与活检联合使用时,251例高位胃癌的阳性率(94％)显著高于单独活检的阳性率(72.1％)。

The analysis of the diagnostic on highexated gastric cancer of 251 cases by the combination use of endoscopic tissue film glass, sliding film' s cellular examination and biopsy.

In current report the clinical and endo-scopic data of 251 cases with high-positioned gastric cancer revealed in 8 096 cases of gastroscopic evaluated patients are reviewed in our two hospitals. These 251 cases with high-positioned gastric cancer constitute 28. 2% (251/890) of the all patients with gastric cancer in concurrent period. Males of above forty predominate 40. 3% cases are of complicating hemorrhagia. The definitive diagnostic rate with eadoscopic naked-eye' s evaluation is 94. 8%, being markedly above its corresponding rate in the finding of X-ray examination, after barium meal (37.9%).The most of these 251 cases are in advanced stage. Only 3 cases (1. 2%) are in early stage. In cytologic examination with the use of film glass, sliding film as well as biopsy combined, the positive rate (94%) in 251 cases with high-positioned gastric cancer is markedly above the corresponding rate (72. 1%) of the method singly operated.The author regards that in endoscopic examination three methods used in combination might increase the accuracy for diagnostic rate of high-positioned gastric cancer and recommends the regular application of low or high positioned U-form inversion manipulation for effectively reducing or avoiding the diagnosis in missing.
