丝裂霉素在慢性泪囊炎鼻内镜下手术中的应用

目的:观察慢性泪囊炎泪囊鼻腔吻合术中应用丝裂霉素的临床疗效。方法:42例病人分为2组,每组均21例,21只眼。2组手术方式相同,丝裂霉素组术中应用0.2g·L-1丝裂霉素棉片置吻合口处10min,术后随访6mo。观察2组手术疗效,用鼻内镜检查吻合口愈合状况。结果:治疗组治愈20只眼,好转1只眼;对照组治愈10只眼,8只眼好转,3只眼无效,2组疗效差异有非常显著意义,P<0.01。未发现有鼻黏膜坏死、感染等并发症发生。结论:丝裂霉素可作为治疗慢性泪囊炎的一种安全、有效的辅助措施,明显提高手术成功率。

Application of mitomycin in chronic dacryocystitis operation under paranasal sinus endoscopy

AIM: To evaluate the curative effect of mitomycin used in dacryocystorhinostomy for treating chronic dacryocystitis. METHODS: Forty-two patients were divided into two groups with 21 patients and eyes for each. The operation methods of the two groups were the same. In the mitomycin group, a piece of cotton soaked with 0.2 g·L~(-1) mitomycin was applied on the anastomotic site and removed 10 min later. Afterward, all patients were followed up for 6 mo. The clinical effects and the concrescence situation of the wounds including patency of lacrimal ducts were closely observed under paranasal sinus endoscope. RESULTS: In the mitomycin group, the symptoms of 20 eyes vanished completely except 1 eye with promotion of the symptom. In the control group, the symptoms of 10 eyes vanished completely, with promotion of symptoms in 8 and invalid for 3. There was significant difference between the two groups (P<0.01). No necrosis of nasal mucosa and infection occurred. CONCLUSION: Mitomycin can be used for an auxiliary measure to treat the chronic dacryocystitis with safe, effective and higher successful rate.
