胃肠道间质瘤临床病理分析(附35例报告)

目的 探讨胃肠道间质瘤临床病理特征及其相关诊断问题。方法 应用组织学和免疫组织化学方法对35例胃肠道间质瘤进行形态学分析。结果 35例中消化道内32例，其中胃2l例(60．0％)、小肠9例(25．7％)、直肠2例(5．7％)；消化道外3例，分别肠系膜、腹膜后、肠系膜及腹膜后混合各1例(8．6％)。免疫组化表达，CD117( )100％、Vim( )97％、CD34( )84．4％、NSE( )54．4％、Aetin( )50％、SMA( )33．3％、S—100( )33．3％、Syn( )16．7％、Des( )12．5％。CD68和CgA(-)。结论 胃肠道间质瘤是胃肠道最常见的间叶性肿瘤，充分认识其病理形态学特点，结合免疫组化CD117的染色，才有利于正确诊断。

Clinicopathologic analysis of gastrointestinal stromal tumors (A report of 35 cases)

Objective To investigate the clinicopathologic features and diagnostic problem of gastrointestinal stromal tumors(GISTs).Methods Thirty five cases of GiSTs morphology were analyzed by histology and immunohistochemistry.Results Thirty two of 35 cases were intra-alimentary canal,the tumors were located in the stomach (n=21)(60.0%),small intestine(n=9)(25.7%),rectum(n=2)(5.7%),3 of 35 cases were extra-alimentary canal,they were average 1 case including mesenterum and retroperitoneum,as well as mixer of the mesenterum and retroperitoneum(8.6%).The tumors were immunoreactive for CD117(+)100%,Vim(+)97%,CD34(+)84.4%,NSE(+)54.5%,Actin(+)50%,SMA(+)33.3%,Syn(+)16.7%,Des(+)12.5%.The expression of CD68 and CgA were negative.Conclusion GISTs are the most common gastrointestinal mesenchymal tumors.Recognizing sufficiently their pathomorphology and combining immunoreactive for CD117 are helpful for right diagnosis.
