颅内动脉瘤的显微手术治疗

目的探讨颅内动脉瘤的影像学检查方法、手术时机的选择、术中注意事项、疗效及其并发症的防治等。方法回顾性分析经显微手术治疗的82例颅内动脉瘤患者的临床资料。结果82例共计92个动脉瘤全部成功夹闭，其中9例术中破裂出血。出院时按GOS评分，恢复良好61例，中残12例，重残6例，植物生存1例，死亡2例。结论DSA和3D—CTA均可有效诊断颅内动脉瘤。选择适当的手术时机以及术者具备娴熟的显微外科操作技术，并注重并发症的防治，可使显微手术治疗颅内动脉瘤安全可靠，并获得良好效果。

Microsurgery for Intracranial Aneurysms

Objective To explore the imaging characteristics, operative opportunity and skill of intracranial aneurysms, the therapeutic： effect of microsurgery on them and the prevention of operative complications in the patients with intracranial aneurysms. Method The Clinical data of 82 patients with intracranial aneurysms who underwent mierosurgery from January, 2007 to December, 2008 were analyzed retrospectively. Results Ninety-two aneurysms were clipped successfully in 82 patients with intracranial aneurysms. The rupture and bleeding of the aneurysms occurred during the surgery, in 9 patients. According to GOS, 61 were recovered well, 12 were moderately disabled, 6 were severely disabled, 1 vegetabaly survived and 2 died. Conclusions Both the DSA＇and 3D-CTA were the effective methods to diagnose intracranial aneurysms. Microsurgical treatment of intracranial aneurysms was safe, reliable and effective, but its opportunity and skill, and treatment and the prevention of the complications should be paid attention to.
