一件式造口袋连接负压吸引装置在大便失禁危重患者中的应用

目的探讨一件式造口袋连接负压吸引装置在大便失禁危重患者中的应用对危重患者大便失禁肛周皮肤保护的效果。方法选择2010年4月～2012年9月本院126例大便失禁的危重患者分为实验组65例和对照组61例。实验组采用一件式造口袋连接负压吸引装置收集粪便并保护肛周皮肤,对照组采用湿润烧伤膏保护肛周皮肤。比较两组患者不同程度肛周刺激性皮炎治疗效果、平均每天护理费用、平均每天护理时间。结果实验组病人Ⅰ度、Ⅱ度、Ⅲ度、Ⅳ度肛周刺激性皮炎治疗效果分别好于对照组（P〈0.05）;实验组平均每天护理费用、平均每天护理时间低于对照组。结论一件式造口袋连接负压吸引装置收集大便失禁危重患者的粪水,能预防肛周皮肤损伤,促进肛周刺激性皮炎的愈合,减轻护理工作量,节约了护理成本,效果满意。

The Application of the One-piece Ostomy Bag Connected with Vacuum Suction Device in the Management of Fecal Incontinence in Critical Patients

Objective To discuss the perianal skin protecting effect by using the one-piece ostomy bag connected with vacuum suction device in fecal incontinence critical patients.Methods From April 2010 to September 2012,126 cases of fecal incontinence critical patients were divided into two groups,the experimental group and the control group,with 65 cases and 61 cases respectively.Experimental group patients adopted a-piece ostomy bags connected with vacuum suction device,and the control group adopted the moist exposed burn ointment to protect skin around the anus.The outcomes of the different levels in patients with perianal irritation dermatitis,the average cost of care and the time consuming of perianal skin care of the two groups were compared.Results The treatment effect of the experimental group patients of Ⅰ,Ⅱ,Ⅲ and Ⅳdegree perianal irritation dermatitis were all better than the control group（P0.05）.The experimental group average cost of care and the average hours of nursing were less than the control group（P0.05）.Conclusions It can prevent perianal skin injury,accelerate healing of anal irritation dermatitis,reduce nursing workload,and save the cost of care of one-piece ostomy bag connected with vacuum suction device to collect the incontinence fecal water in critically ill patients,and is illuminated as effective.
