外伤性白内障不同时机手术效果的分析

为探明不同时机手术摘除外伤性白内障和IOL植入的效果 ,对 6 9例眼外伤患者进行回顾性分析 ,按伤后施行手术时间迟早分两组 ,手术在Zeiss手术显微镜下进行 ,根据晶体囊破损与核软硬程度 ,运用超声乳化吸出或挽核技术。后囊完整或有破口 ,但足以支撑IOL者 ,植入PC -IOL ,前后囊破口大 ,则植入AC -IOL。结果Ⅰ组术后矫正视力≥ 0 .4～ 0 .5者占 5 5 .17% ,Ⅱ组占 37.5 % ,明显低于Ⅰ组 ,两组比较差异有显著性 ,P <0 .0 5。其原因可能与伤后开盲时机与眼部并发症有关。认为妥当处理伤口 ,有效控制炎症 ,及时手术摘除白内障和IOL植入 ,可缩短病程 ,加快创伤愈合和视力恢复

Different surgical times causes different results in the treatment of traumatic cataract

In order to investigate different results caused by different surgical times in the extraction of traumatic cataract and the effect of Intra Ocular Lens (IOL) implantation, 69 patients with eye injuries were retrospectively analysed. All the cases were divided into two groups according to the surgical times (Group Ⅰ, 6 days～3 months after injury; Group Ⅱ, 6 months～20 years after injury ). The extraction of cataract were carried out under a Zeiss operating microscope and whether the ultraemulsifying or aspiration technique was chosen to use, it would depend on the rupture of the lens capsule and on the soft or hard degree of nucleus. If the posterior capsule was intact or the lesion could support IOL, the PC-IOL would be implanted, but a AC-IOL would be implanted in a big lesion of the anterior or posterior capsule. Results showed that the corrected vision (≥0.4～0.5) was 55.17% in Group Ⅰ, but Group Ⅱ was 37.5% which was obviously lower than Group Ⅰ and the difference was statistically significant (P<0.05) after operation. This may be related to the time of removing the dressing and the eye complication. It can be concluded if the lesions are treated properly, the inflammation controlled, cataract extracted and IOL implanted in time, then the disease course would be shortened with quicker wounds healing and vision recovery.
