非淋菌性泌尿生殖道感染支原体检测及药敏试验

摘    要:目的:了解解脲脲原体(Uu)和人型支原体(Mh)在非淋菌性泌尿生殖道感染中的致病作用及其体外药物敏感状况.方法:296例疑为非淋菌性泌尿生殖道感染的患者,运用微量稀释法进行支原体培养,测定解脲脲原体对10种抗菌药物的敏感性. 结果:101例(34.1%)支原体培养阳性,其中Uu阳性75例(25.3%),Mh阳性6例(2.0%),Mh+Uu阳性20例(6.8%),女性(42.9%)支原体阳性率显著高于男性(28.8%).Uu对螺旋霉素均耐药,对氧氟沙星、四环素的耐药率均为37.3%,对交沙霉素的耐药率最低(8.0%), 对其他抗菌药物的耐药率依次为美满霉素(33.3%)、强力霉素(32.0%)、红霉素(30.7%)、左氧氟沙星(18.7%)、阿奇霉素(13.3%)、罗红霉素(10.7%).Uu对四环素类、大环内酯类和喹诺酮类有交叉耐药,以对四环素类和喹诺酮类的交叉耐药率最高(12.0%).结论:支原体的定期耐药性监测对临床用药有重要意义.

Detection of Mycoplasma in 296 Patients with Non-gonococcal Urogenital Tract in Fection and Test of Susceptibility of Ureaplasma Urealytica to Antimicrobial Agents

Abstract:Objective:To explore the role of Ureaplasma urealytica (Uu) and Mycoplasma hominis (Mh) in non gonococcal urogenital tract infection and examine their in vitro drug susceptibility. Methods:Detection of mycoplasma and susceptibility test of ureaplasma urealytica to 10 antimicrobial agents were performed by using microdilution method. Results:Among the 296 suspected cases, 101(34.1%) were positive with mycoplasma , 75 (28.8%) with Uu, 6(2.0%) with Mh, and 20 (6.8%) with Mh plus Uu. The positive rate with mycoplasma infection was significantly higher among the female subjects (42.9%) than that among the male (28.8%). The drug susceptibility test showed that, among the 10 antimicrobial agents tested, resistance of Uu was most commonly found to spiramycin (100.0%), and least found to josamycine (7.9%). Frequency of resistance of Uu, among the tested cases, to the other antimicrobial agents, was as followes: to ofloxacin (37.3%), to tetracycline (37.3%), minocycline (33.3%), doxycycline (32.0%), erythromycine (30.7%), levofloxacin (18.7%), azithromycin (13.3%), and to roxithromycin (10.7%). The cross resistance to fluoro quinolones , tetracycline and macrolides was found. The highest rate of cross resistance was between fluoro quinolores and tetracycline (12.0%). Conclusion:Monitoring the variance of resistance of mycoplasma is of important significance for guiding the use of drugs.
