乳腺癌保乳手术与改良根治术50例临床比较分析

目的 比较分析乳腺癌保乳手术与改良根治术的临床治疗效果.方法 选择2004年6月至2011年5月50例早期乳腺癌保乳手术患者与50例同期行改良根治术患者的资料进行回顾性对比分析,并随访1～3年.结果 经两种手术方法治疗后,两组早期乳腺癌患者1、3年生存率均为100％,保乳手术组复发及转移9例,占18.0％,改良根治术组复发及转移8例,占16.0％,两组比较差异无统计学意义(P＞0.05).但在美容效果上保乳手术优于改良根治术,50例患 者中31例评判结果为优,15例为良,优良率达92.0％.结论 乳腺癌保乳手术与改良根治术均能有效治疗乳腺癌,患者的疗效及生存期相比无明显差异,但保乳手术在美容效果及对患者的心理影响方面较改良根治术具有明显优势.因此,在严格掌握手术指征的前提下应尽量选择保乳手术.

Analysis of clinical comparison for breast conserving surgery and modified radical mastectomy in 50 cases

Objective To compare and analyze the clinical effects of breast conserving surgery and modified radical mastectomy.Methods 50 cases of breast-conserving surgery for early breast cancer and 50 cases of modified radical mastectomy at the same period in our hospital from June 2004 to May 2011 were analyzed retrospectively and comparatively,and follow-up for 1-3 years.Results After two kinds of surgical treatment,the 1-and 3-year survival rates for the two groups of early breast cancer were 100%.In the breast-conserving surgery group,recurrence and metastasis were in 9 cases,accounting for 18.0%,while in the modified radical mastectomy group,recurrence and metastasis were in 8 cases,accounting for 16.0%.There was no statistical difference between the two groups(P>0.05).However,the cosmetic results of breast-conserving surgery were superior to the modified radical mastectomy.Among 50 cases in this group,31 cases were judged as excellent,15 cases as good,with the excellent rate of 92.0%.Conclusion Breast-conserving surgery and modified radical mastectomy for breast cancer can be effective in treating breast cancer.The efficacy and survival has no significant difference,but breast-conserving surgery has obvious advantages in the beauty and the psychological impact than modified radical mastectomy.Therefore,on the premise of strict control of surgical indi cations,the breast-conserving surgery should be selected as possible.
