糖尿病合并脑梗塞患者颈动脉硬化斑块形态研究

目的 研究糖尿病合并脑梗塞患者颈动脉粥样硬化斑块形态及斑块形态与脑梗塞发生的关系。方法 应用彩色多普勒超声诊断仪对156例糖尿病患者颈动脉超声检查，根据CT或者MRl分为糖尿病合并脑梗塞组(DMCI)、糖尿病非脑梗塞组(DMNCI)，检测分析二组颈动脉中内膜厚度(IMT)、内径(D)、斑块发生率及斑块形态。结果 DMCI组IMT、D、斑块发生率明显高于DMNCI组，有显著性差异性(P＜O．05，P＜0．01)；不稳定的软斑、溃疡斑、混合斑发生率增加(P＜O．05)。结论 颈动脉粥样斑块是造成脑梗塞的主要原因。不稳定性斑块是脑梗塞的独立危险因素。颈动脉超声能显示动脉粥样硬化的相关物理特性，对糖尿病患者的脑梗塞预防及治疗具有重要意义。

Study on carotid plaque morphology in patients with diabetic mellitus combined with cerebral infarction

Objective To study the carotid plaque morphology in patients with d iabetic mellitus combined with cerebral infarction. Methods The carotid arteries of 156 diabetic patients were examined by color doppler sonography. According t o CT or MRI results patients with diabetic were divided into two groups: one of diabetic mellitus with cerebral infarction (DMCI), the other of diabetic mellitu s not with cerebral infarction( DMNCI). Carotid artery intima-media thickness ( I MT), diameter(D), the incidence of carotid plaque and plaque morphology were m easured, respectively. Results The carotid artery IMT, D, and incidence of carot id plaque in DMCI group were higher than that in DMNCI group(P<0.05,P<0. 01). T he occurrence rates of unstable atherosclerotic soft plaque, ulcer plaque and mi xing plaque increased in DMCI group(P<0.05). Conclusions The carotid atheroscl erotic plaque is the main reason for cerebral infarction; unstable plaque is an independent risk factor for it. Carotid artery sonography can show physical char acteristic of carotid artery. It is very important for patients with diabetic me llitus to prevent or treat their illness of cerebral infarction. <
