摘    要:目的 探讨医疗失效模式与效应分析(HFMEA)在供应室器械回收管理中的应用效果。 方法 根据HFMEA管理方法,成立器械回收管理小组,分析器械回收中存在的风险,明确失效模式及原因,制定针对性的措施解决主要失效因素。比较HFMEA实施前后各失效模式风险指数(RPN)值的变化和不良事件发生情况。 结果 HFMEA实施后各失效模式RPN评分较实施前降低;实施后回收人员器械伤发生率、清点数目不符率、回收登记错误率和科室初步清洗不合格率均较实施前降低。 结论 HFMEA管理方法能够及时发现供应室器械回收环节的失效模式,有效降低失效模式所带来的影响,促进临床工作质量的提升。

Abstract:Objective To explore the effect of application of healthy failure mode and effect analysis (HFMEA) in instrument recycling management of the supply room. Method The management group for recycling of instruments was established according to the HFMEA management method. Risks existing in recycling of instruments were analyzed. Failure modes and corresponding causes were clarified. And targeted measures were developed regarding the main failure factors. The changes of the risk priority number (RPN) of all failure modes and occurrence rate of adverse events were compared before and after the HFMEA implementation. Results After the implementation of HFMEA, the RPNs of all failure modes were significantly lower than those before the intervention (P<0.05). The occurrence rate of instrument injury among recycling staff. Discrepancy rate of physical inventory, error rate of recycling records and unqualified rate regarding preliminary cleaning of relevant departments after implementation were all significantly lower than those before implementation (P<0.05). Conclusion Timely detection of failure modes in recycling of instruments from the supply room could be achieved using the HFMEA management method, which could effectively decrease the effect brought by failure modes and promote the quality of clinical service
