儿童联合性垂体激素缺乏症临床特点观察

目的研究儿童联合性垂体激素缺乏症(CPHD)的临床特点,总结诊疗经验。方法对24例儿童CPHD患者资料进行临床特点、疗效分析。结果 24例患者中,生长激素缺乏24例,促甲状腺激素缺乏13例,促肾上腺皮质激素缺乏19例;15例行促性腺激素释放激素类似物刺激试验,提示促性腺激素缺乏13例。激素替代治疗后病情好转,1例发生垂体危象。结论身材矮小的患者,需行系统的下丘脑-垂体-靶腺轴功能及相关影像学检查,以明确诊断。有异常出生史的矮小患者是CPHD的高发人群。

An investigation on the clinical features of combined pituitary hormone deficiency of children patients

Objective To investigate the clinical features of children CPHD patients,and to summarize the experience of the diagnosis and treatment.Methods Twenty-four cases of children CPHD were analyzed,including the clinical features and the treatment effects,experience of the diagnosis and treatment obtained as well.Results Of twenty-four cases,all were suffering from deficiency of at least two kinds of pituitary hormone.Patients' conditions improved after hormone treatment.Pituitary crisis happened on one case.Conclusion Systematic examinations of hypothalamic-pituitary-target gland axis are needed for those children who give the expression of short stature,to establish the diagnose of CPHD.Those with abnormal birth history are of greater risk of CPHD.
