大黄对重症急性胰腺炎患者血清IL-15IL-18影响的研究

目的探讨大黄对重症急性胰腺炎(SAP)患者血清白细胞介素-15(IL-15)、白细胞介素-18(IL-18)的影响。方法 58例SAP患者随机分为A组(n=22)、B组(n=36)。A组只行综合治疗,B组在综合治疗基础上给予鼻空肠管注入大黄。ELISA检测患者血清入院时(0 d)、7 d、14 d时IL-15、IL-18水平。以健康体检人群血清作为对照。结果入院0 d时A、B组IL-18均显著高于对照组(P<0.01),IL-15均显著低于对照组(P<0.01),但IL-18、IL-15在A、B组之间相互比较差异均无统计学意义(P>0.05);治疗7 d时A、B组IL-18均比入院时下降,其中以B组下降最明显;A、B组之间IL-18相互比较差异有统计学意义(P<0.01)。治疗7 d时A组IL-15与入院时比较差异无统计学意义,但B组IL-15明显升高,A、B组之间比较差异有统计学意义(P<0.01)。治疗14 d时A、B组IL-18、IL-15与治疗7 d时比较,差异均无统计学意义(P>0.05),而且,在综合治疗基础上给予大黄治疗后,B组总有效率均明显高于A组,差异有统计学意义(P<0.05)。结论通过调节炎症介质IL-15、IL-18的表达,大黄能有效地逆转SAP病情。

Effects of rude rhubarb on serum IL-15 and IL-18 levels in patients with severe acute pancreatitis

Objective To explore the effects of rode rhubarb on serum interleukin-15(IL-15),interleukin-18(IL-18)levels in patients with severe acute panereatitis(SAP).Methods SAP patients were divided into group A(n=22),B(n=36) randomly.The patients in group A were given comprehensive treatment.On the basis of comprehensive treatment,the patients in group B were given rude rhubarb by nasojejunal feeding.The serum levels of IL-15 and IL-18 in two groups were detected by ELISA assay at diferent intervals(0 d,7 d,14 d).The serum of the healthy volunteers was acted as control.Results On day 0,the serum levels of IL-18 in A,B group were higher than those of the control(P <0.01),and the serum levels of IL-15 in A,B group were lower than those of the control(P <0.01).However,the serum levels of IL-18 and IL-15 between A,B group did not show significant differences(P>0.05,respectively);On day 7,the serum levels of IL-18 in A,B group decreased compared with those on day 0(obvious decrease was observed in group B);in addition,the levels of IL-18 between A,B group showed significant differences(P <0.01);On day 7,the serum levels of IL-15 in A group did not change obviously compared with those on day 0,but IL-15 in B group was significant increased compared with that on day 0(P <0.01).On day 14,the serum levels of IL-18,IL-15 in A,B group did not change obviously ccompared with those on day 7(P >0.05,respectively).Moreover,the total effective rate in group B was higher than that of A(P <0.05)after administration of rude rhubarb.Conclusion Administration of rude rhubarb can effectively reverse SAP by regulating the levels of IL-10 and IL-18.
