TGF-β1、TNF-α、IL-6与多囊卵巢综合征发病关系的探讨

摘    要:目的:探讨TGF-β1、TNF-α、IL-6与多囊卵巢综合征(PCOS)发病的关系。方法:酶联免疫吸附试验(EusA)检测血清TGF-β1、TNF-α、IL-6水平,放射免疫法(RIA)检测血清胰岛素水平,葡萄糖氧化酶法检测血糖水平,化学发光仪检测血清甾体激素水平。结果:(1)TGF-β1水平在PCOS组高于对照组,在两组中与其他指标均无相关性;TNF-α水平在PCOS组明显高于对照组,且与IL-6水平呈明显正相关,与胰岛素敏感指数(ISI)呈明显负相关,与睾酮(T)和雄烯二酮(A2)呈正相关,在两组中均与BMI呈正相关;IL-6水平在PCOS组明显高于对照组,且与BMI呈正相关,与空腹胰岛素(FIN)水平呈明显正相关,与ISI呈明显负相关,对照组中与各项指标无相关性。(2)PCOS组有IR者TNF-α、IL-6、FIN明显高于无IR者。结论:TGF-β1、TNF-α、IL-6都与PCOS的发病有关,TGF-β1可能主要影响胰岛素的敏感性和糖代谢,TNF-α、可能主要通过影响外周组织对胰岛素的敏感性和导致局部雄激素水平升高来发挥作用,IL-6主要与IR有关,TNF-α和IL-6有协同作用,能加重PCOS病情的发生发展。

Explore on the relationship between transforming growth factor-beta1, tumour necrosis factor-alpha and interleukin-6 and the pathogenesis of polycystic ovary syndrome
Abstract:Objective To explore the relationshi p between transforming growth factor-beta1, tumo ur necrosis factor-alpha and interleukin-6 and the pathogenesis of polycystic ovary syndrome(PCOS).Methods From Feb. 2003 to June 2003, 20 patients with PCOS (PCOS group) and 20 normal re productive age female (control group) were studied in this experiment. Serum lev e ls of transforming growth factor-beta1, tumour necrosis factor-alpha and inter leukin-6 were determined by ELISA,Serum insulin levels by radioimmunoassay,gl ucose levels by glucose oxidase method. Results (1)Serum levels of transforming growth factor-beta1 i n PCOS group were higher than those in control group,but had no correlation wit h certain indices in both groups;serum levels of tumour necrosis factor-alpha in PCOS group were significantly higher than those in control group,related to both testosterone and androstenedione positivel,negatively related to insulin s ensitivity index significantly,positively related to body mass index in both gr oups;serum levels of interleukin-6 in PCOS group were significantly higher tha n those in control group,positively related to body mass index,negatively rela ted to insulin sensitivity index significantly,positively related to fasting in sulin levels and serum levels of tumour necrosis factor-alpha significantly. (2 ) Serum levels of tumour necrosis factor-alpha, interleukin-6, fasting insulin w ere significantly higher in PCOS group with insulin resistance than those withou t insulin resistance. Conclusion Transforming growth factor-beta1 may induce polyc ystic ovary syndrome by affecting the metabolism of insulin and glucose. Tumour necrosis factor-alpha relates closely to both insulin resistance and hyperandro genism,while interleukin-6 relates predominantly to insulin resistance,tumour necrosis factor-alpha and interleukin-6 may have coordinate functions.
