摘    要:目的 探讨长托宁联合呼气末正压通气模式治疗重度急性有机磷农药中毒患者的临床疗效.方法 治疗组采用长托宁联合呼气末正压通气模式治疗30例重度急性有机磷农药中毒患者,并与对照组传统方法治疗的30例患者进行比较,动态检测胆碱酯酶(CHE)活性,观察长托宁用量、昏迷时间、机械通气时间、病死率、住院时间及并发症发生率.结果 与对照组比较,治疗组CHE恢复时间、昏迷时间、机械通气时间、并发症发生率和病死率明显降低(P<0.05),治愈率明显升高(P<0.05).结论 长托宁联合呼气末正压通气模式治疗重度急性有机磷农药中毒疗效确切,值得临床推广.

Abstract:Objective To observe the therapeutic effects of positive end expiratory pressure combined with penehyclidine hydrochloride on severe acute organophosphorus pesticide poisoning.Methods 30 patients with severe acute organophosphorus pesticide poisoning were treated by penehyclidine hydrochloride combined with positive end expiratory pressure(treatment group),and compared with 30 patients with severe acute organophosphorus pesticide poisoning treated by routine therapy(control group).The activity of choline esterase(CHE),dosage of penehyclidine hydrochloride,during of coma,time of mechanical ventilation,mortality,hospitalization time and incidence of complications were detected and compared between two groups.Results As compared with those of control group,the activity of CHE,during of coma,time of mechanicalventilation,mortality and incidence of complications in treatment group were significantly decreased(P<0.05),whereas the cure rate was obviously increased(P<0.05).Conclusion The penehyclidine hydrochloride combined with positive end expiratory pressure has a definite therapeutic effect on severe acute organophosphorus pesticide poisoning,which is worth to be spread in clinic.
