Orem 自护理论在骨科手术切口感染老年患者中的应用效果

目的：探讨Orem自护理论在骨科手术切口感染老年患者中的应用效果。方法将2010年1月-2012年10月收治的骨科手术老年患者120例，随机分为观察组60例和对照组60例。对照组采用常规护理方法，观察组在常规护理方法的基础上采用Orem自护理论的3个护理体系帮助患者逐渐实现自我护理。比较两组患者手术切口感染率和护理满意度。所得数据采用SPSS 16．0统计软件进行分析，患者满意度比较采用t检验，切口感染发生率比较采用χ2检验，以P＜0．05为差异有统计学意义。结果对照组患者满意度评分为（84．23±177;5．52）分，观察组为（97．98±177;6．01）分，差异具有统计学意义（t＝4．980，P＜0．05）。对照组有3例发生切口感染，观察组无一例发生切口感染，差异有统计学意义（χ2＝12．412，P＜0．05）。结论 Orem自护理论在骨科手术切口感染护理中的应用能够帮助患者取得良好的手术效果，值得在临床推广。

Application effect of Orem self-care theory on incision infection in the elderly patients in the orthopedics department

Objective To explore the application effect of Orem self -care theory on incision infection in the elderly patients in the orthopedics department .Methods One hundred and twenty elderly patients with operation in the orthopedics department were randomly divided into the observation group and the control group , each with 60 cases.The control group received the routine nursing , and the observation group received the self-care with the help of Orem self-care theory including three systems on the basis of routine nursing .The incidence rate of incision infection and the satisfaction of nursing were compared between the two groups .The＆amp;nbsp;obtained datas were analyzed by the statistical software SPSS 16.0, the satisfaction of patients was compared with t test, the incidence rate of incision infection was compared with χ2 test, and a P-value less than 0.05 was considered statistically significant .Results The score of patients ’ satisfaction was （84.23 ±177;5.52） in the control group, and （97.98 ±177;6.01） in the observation group, and the difference was statistically significant （t=4.980, P＆lt;0.05）.The cases of incision infection were three cases in the control group , and no incision infection was found in the observation group, and the difference was statistically significant （χ2 =12.412, P ＆lt;0.05）. Conclusions Orem self-care theory can help the elderly patients with incision infection get better result of operation in the orthopedics department .
