CT诊断外伤性小脑幕及大脑镰硬膜下血肿的应用价值及鉴别诊断情况

目的探讨CT诊断外伤性小脑幕及大脑镰硬膜下血肿的应用价值及鉴别诊断情况。方法选择2015年6月12日~2016年10月20日至我院就诊的外伤性小脑幕及大脑镰硬膜下血肿患者98例作为本次研究对象,均采用CT检查,对其检查结果进行综合分析。结果 98例患者经过CT检查后得知,单侧病变57例,双侧病变41例;其中单纯大脑镰血肿14例,小脑幕血肿14例,小脑幕及大脑镰血肿16例,小脑幕及大脑镰血肿伴有蛛网膜下腔出血17例,小脑幕及大脑镰血肿伴有颅骨下方颅脑凸面硬膜下血肿10例,脑挫裂伤且脑内血肿合并大脑镰、小脑幕急性硬膜下血肿27例。该类疾病患者以条带状、条索状高密度影为主要的影像学特征;新月形、片状可为小脑幕硬膜下血肿形状;诊断准确性为92.86%。结论对外伤性小脑幕及大脑镰硬膜下血肿患者采取CT诊断的应用价值较高,临床上应做好外伤性小脑幕及大脑镰硬膜下血肿的鉴别诊断工作,以免与其他疾病相互混淆,影响最终的诊断结果。

Application value and differentiated diagnosis of CT in the diagnosis of traumatic tentorium cerebelli and cerebral falx subdural hematoma

Objective To investigate the application value and differential diagnosis of CT in the diagnosis of traumatic tentorium cerebelli and cerebral flax subdural hematoma.Methods From June 12,2015 to October 20,2016,98 patients with traumatic tentorium cerebelli and cerebral falx subdural hematoma who were admitted to our hospital were selected as our study subjects.CT was applied for examination.The results of examination were analyzed comprehensively.Results After CT examination,among 98 patients,unilateral lesion was in 57 cases,and bilateral lesion was in 41 cases;among them,simple cerebral falx hematoma was in 14 cases,and tentorium cerebelli hematoma was in 14 cases.There were 16 cases with tentorium cerebelli and cerebral falx hematoma.There were 17 cases with tentorium cerebelli and cerebral falx hematoma complicated with subarachnoid hemorrhage,and there were 10 cases with tentorium cerebelli and cerebral falx hematoma complicated with cranial subdural hematoma at convex side under the cranium.There were 27 cases with cerebral contusion and brain hematoma complicated with tentorium cerebelli and cerebral falx acute subdural hematoma.For these patients,the main imaging features were strip and cord-like high-density shadows;crescent shape and flaky shape can be the shape of tentorium cerebelli subdural hematoma;the diagnostic accuracy was 92.86%.Conclusion CT diagnosis of tentorium cerebelli and cerebral falx subdural hematoma is of high application value.The differentiated diagnosis of traumatic tentorium cerebelli and cerebral falx subdural hematoma should be carried out clinically,so as not to be confused with other diseases,and avoid affecting the final diagnosis.
