保乳术与改良根治术治疗早期乳腺癌的临床疗效比较

目的探讨保乳术与改良根治术治疗早期乳腺癌的临床疗效。方法选取2009年1月~2012年9月期间我院收治的80例早期乳腺癌患者作为研究对象,将其分成研究组(n=40)与对照组(n=40),其中研究组患者接受保乳手术,对照组患者接受改良根治术。对比两组患者的手术相关指标(术中出血量、手术时间、手术切口长度以及平均住院天数)、乳房外观优良率、并发症发生率、术后生活质量评分、术后5年的复发率及生存率。结果研究组的术中出血量、手术时间、手术切口长度、平均住院天数、乳房外观优良率、并发症发生率、生活质量总分均明显优于对照组,差异具有统计学意义(P0.05)。两组患者5年内的复发率及生存率比较,差异均无统计学意义(P0.05)。结论保乳术治疗早期乳腺癌的创伤小,术后恢复情况、乳房外观情况及生活质量总体情况均好于改良根治术,值得临床推广应用,但在临床推广时应注意对患者的心理疏导,减轻其对保乳手术复发的顾虑。

Comparison of the clinical efficacy of breast conserving surgery and modified radical mastectomy in the treatment of early breast cancer

Objective To explore the clinical efficacy of breast conserving surgery and modified radical mastectomy in the treatment of early breast cancer.Methods 80 patients with early breast cancer who were admitted to our hospital from January 2009 to September 2012 were selected as the study subjects.They were assigned to the study group (n=40)and the control group(n=40),in which the study group was given breast conserving surgery,and the control group was given modified radical mastectomy.The surgical related indicators(intraoperative blood loss,operation time,length of surgical incision and average length of hospital stay),excellent and good rate of breast appearance,incidence rate of complications,postoperative quality of life scores,postoperative 5-year recurrence rate and survival rate were compared between the two groups.Results The intraoperative blood loss,operation time,incision length,average length of hospital stay,excellent and good rate of breast appearance,incidence rate of complications and quality of life scores in the study group were significantly better than those in the control group(P<0.05),and the differences were statistically significant.There was no statistically significant difference in the 5-year recurrence rate and survival rate between the two groups(P>0.05).Conclusion Breast conserving surgery in the treatment of early breast cancer has small trauma.Postoperative recovery,breast appearance and overall quality of life are better than improved radical surgery,which is worthy of clinical promotion and application.However,in clinical promotion,attention should be paid to the psychological counseling of patients,so as to reduce their concerns about the recurrence of breast-conserving surgery.
