HBV相关慢加急性肝功能衰竭患者外周血单核细胞CD163和CD69的表达及其与病情、预后的关系

目的了解HBV相关慢加急性肝功能衰竭（HBV-ACLF）患者外周血单核细胞CD163和CD69的表达情况,分析其与患者病情、预后的关系。方法应用流式细胞术对HBV-ACLF患者30例、乙型肝炎肝硬化25例、慢性乙型肝炎18例、健康对照15例外周血单核细胞CD163与CD69的表达情况进行检测,结合肝功能衰竭患者病情及预后进行分析。结果 HBV-ACLF组外周血单核细胞表面CD163的表达率低于肝硬化组、慢性乙型肝炎组及健康对照组（P〈0.05）,随肝功能衰竭早、中、晚期病情进展,CD163的表达率逐渐降低（F=10.921,P=0.000）,死亡组CD163的表达率明显低于存活组（P〈0.05）;HBV-ACLF组CD69的表达率高于肝硬化组、慢性乙型肝炎组及健康对照组（P〈0.05）,随肝功能衰竭早、中、晚期病情进展,CD69的表达率逐渐增高（F=11.504,P=0.000）,死亡组CD69的表达率明显高于存活组（P〈0.05）;CD163和CD69评估HBV-ACLF患者3个月内死亡情况所对应的受试者工作特征（ROC）曲线下面积（AUC）分别为0.726、0.735。结论 HBV-ACLF患者外周血单核细胞CD163和CD69的表达与病情严重程度及预后密切相关。

The expression of CD163 and CD69 in patients with HBV related acute-on-chronic liver failure and its association with disease development and prognosis

Objective To investigate the role of CD163 and CD69in patients with HBV related acute-on-chronic liver failure（HBV-ACLF）and its association with disease development and prognosis.Methods Peripheral blood mononuclear cells were separated from30 HBV-ACLF patients（HBV-ACLF group）,25 liver cirrhosis patients（LC group）,18 chronic hepatitis B patients（CHB group）and 15 normal persons（normal group）.The expression of CD163 and CD69 was detected by flow cytometry.Results The expression of CD163in HBV-ACLF group was lower than that of LC group,CHB group and normal group（P0.05）;The expression of CD163 reduced with the development of disease（F=10.921,P=0.000）; In the dead group,the expression of CD163 was obviously lower than that in survival group（P0.05）.The expression of CD69 in HBV-ACLF group was higher than that of other groups（P〈0.05）,and it was associated with the development of disease（F=11.504,P=0.000）;The expression of CD69 in dead group was obviously higher than that in survival group（P〈0.05）.The association of CD163 and CD69 with prognosis in HBC-ACLF patients in3 months was evaluated via ROC analysis,the area under curve（AUC）were 0.726 and0.735,respectively.Conclusion The expression of CD163 and CD69 was associated with disease development and prognosis in HBV-ACLF patients.
