细胞周期增殖指标在甲状腺乳头状癌中的诊断价值

摘    要:目的:对比研究细胞周期增殖指标MCM7和Ki-67蛋白在人甲状腺乳头状癌中的诊断价值。方法应用免疫组织化学SP法对甲状腺乳头状癌40例、腺瘤30例、结节性甲状腺肿30例及正常甲状腺组织20例进行MCM7、Ki-67蛋白表达检测。结果 MCM7、Ki-67蛋白在乳头状癌中的阳性表达率为100.00%(40/40)、80.00%(32/40),均显著高于良性病变及正常组中的表达率( P均<0.01)。甲状腺乳头状癌组织MCM7阳性表达率高于Ki-67,差异有统计学意义( P<0.01)。乳头状癌中MCM7、Ki-67蛋白表达与患者的性别、年龄、肿瘤直径及淋巴结转移情况等临床病理参数均无相关性( P均>0.05)。癌症组织中MCM7与Ki-67的表达呈正相关(r=0.551,P<0.05)。结论甲状腺细胞周期紊乱、细胞异常增殖状态下出现的MCM7、Ki-67蛋白高表达在可能与甲状腺乳头状癌的发生有关,两者联合检测或许可作为辅助诊断甲状腺乳头状癌的一项参考指标。MCM7在评价甲状腺恶性肿瘤细胞增殖活性上优于Ki-67,对评价肿瘤细胞增殖状态有一定意义。

Abstract:Objective To explore the value of minichromosome maintenance protein 7 ( MCM7 ) and Ki-67 proteins in the proliferation biomarkers of cell cycle in the diagnosis of papillary thyroid carcinoma .Methods Immu-nohistochemical SP method was used to detect the expressions of MCM 7 and Ki-67 proteins in 40 cases of papillary thyroid carcinoma,30 cases of thyroid adenoma ,30 cases of nodular goiter and 20 cases of normal thyroid tissue . Results The positive rates of MCM7 and Ki-67 expressions in papillary thyroid carcinoma were 100.00%(40/40) and 80.00%(32/40),respectively,which were significantly higher than those in thyroid adenoma ,nodular goiter and normal thyroid tissue(all P〈0.01).In papillary thyroid carcinoma ,the positive rate of MCM7 expression was significantly higher than that of Ki-67 expression(P〈0.01).The expressions of MCM7 and Ki-67 proteins in papillary thyroid carcinoma were not related with patients′gender,age,tumor diameter and lymph node metastasis (all P〉0.05).The expression of MCM7 protein had a positive correlation with the expression of Ki-67 protein in papillary thyroid carcinoma tissue (r=0.551,P〈0.05).Conclusion The high expressions of MCM7 and Ki-67 protein in the thyroid of patients with cell cycle disorder and abnormal cell proliferation may involve in the development of papillary thyroid carcinoma .The combined examination of MCM 7 and Ki-67 proteins may be used as a biomarker in the diagnosis of papillary thyroid carcinoma.MCM7 is superior to Ki-67 in evaluating the activity of cell proliferation of malignant thyroid tumor ,and it has certain significance on evaluating the proliferation of tumor cells .
