丝裂霉素C结膜下注射联合那素达滴眼液在翼状胬肉手术中的临床应用

摘    要:目的:探讨术前低浓度丝裂霉素C(MMC)结膜下注射联合术后那素达(盐酸萘甲唑啉马来酸非尼拉敏)滴眼液治疗翼状胬肉的临床疗效。方法:将翼状胬肉随机分为3组,A组(实验组):术前低浓度MMC结膜下注射联合术后那素达滴眼组68例(80只眼);B组(对照组1):翼状胬肉切除联合MMC及羊膜移植术组64例(77只眼);C组(对照组2):翼状胬肉切除联合MMC及自体角膜缘干细胞移植术组63例(73只眼)。术后1周,2周,1月,3月,1年,2年检查记录术眼并发症及复发情况。结果:随访观察6-24个月,A组3只眼复发(3.75%);B组10只眼复发(12.99%);C组9只眼复发(12.33%)。A组与B组、C组对比差异有统计学意义(P<0.05),B组与C组对比差异无统计学意义(P>0.05)。结论:术前低浓度MMC结膜下注射联合术后那素达滴眼并发症少,可有效预防翼状胬肉复发。

Clinical Application of Mitomycin C plus Naphcon-A in Excision of Pterygium
Abstract:Objective:To explore clinical values of Mitomycin(MMC)under low concentration in excision of pterygium with Naphcon-A.Methods:Patients with primary pterygium were randomized into three groups.Group A(experiment group,80eyes of 68patients)received the preoperative MMC under low concentration with Naphcon-A.Group B(control group 1,77eyes of 64patients)received MMC in excision of pterygium with the amniotic membrane transplantation. Group C(control group 2,73eyes of 63patients)received MMC in excision of pterygium with limbal epithelial autograft transplantation.Recurrence and complication of all cases were monitored after surgery in one week,two weeks,one month,there months,one year,and two years. Results:After clinical follow-up for 6-24months,three eyes of the 80eyes in the group A had recurrence(3.75%).Ten eyes of the 77eyes in group B had recurrence(12.99%).Nine eyes of 73 eyes in group C had recurrence(12.33%).And difference in recurrence rate was found between experimental group and control groups(P<0.05).Conclusion:In order to effectively prevent of pterygium from relapse and reduce complications,it is important to use preoperative MMC under low concentration with Naphcon-A in excision of pterygium.
