106例腹腔镜子宫切除术临床分析

摘    要:目的探讨腹腔镜子宫切除术临床效果。方法选择2011年9月—2013年3月我院收治的腹腔镜(LAP)下全宫切除术106例临床资料进行回顾性分析。结果 106例在腹腔镜下完成手术,无中转开腹。71例单纯全宫切除,10例同时行附件切除,25例同时行卵巢囊肿剔除,25例行盆腔粘连松解术。平均手术时间(174.8±57.3)min,术中出血量为(102.9±89.0)ml,子宫平均重量(260.0±125.4)g,术后排气时间平均为(31.7±8.7)h,术后平均抗炎时间为(3.9±1.2)天,术后平均住院时间为(5.7±1.0)天;无术中术后并发症发生。结论腹腔镜下全宫切除术具有损伤少恢复快等优点,值得临床推广应用,但要严格掌握适应症。

Abstract:Objective To investigate the clinical effectiveness of total laparoscopic hysterectomy (TLH). Methods We analyzed the clinical records of 106 cases that underwent TLH in our hospital from September 2011 to March 2013. Results The laparoscopic hysterectomy was completed in all of the 106 cases without conversion to open surgery. Among the patients, sim- ple hysterectomy was performed on 71 cases, while the following procedures were carried out simuhaneously in another 35 cases: unilateral or bilateral adnexectomy (10 cases), oophorocystectomy (25 cases), adhesiolysis (25 cases). The mean operation time was ( 174. 8 ±57.3) rain, and intraoperative blood loss was ( 102. 9±89.0) ml, the mean weight of uterus was (260. 0 ± 125.4) g, the anal exhaust time was (31.7 ±8.7) h, the anti-infection treatment after operation was adopted for (3.9±1.2) d, and the mean hospital stay was (5.7± 1.0) d. No complications occurred during and after the treatment. Conclu- sion Total laparoscopic hysterectomy has advantages of micro-invasion, rapid recovery and so on. It is worth clinically applied, but in the meanwhile it must he strict to grasp clinical indications.
