胆囊切除术中胆管造影的临床应用与评价

目的：评价胆囊切除术中胆管造影的临床价值。方法：回顾性分析1993年10月。2001年10月因胆囊结石行胆囊切除术患者737例。结果：胆囊切除后根据造影结果再行胆总管探查312例，胆总管结石阳性率94．1％；胆囊切除后直接行胆总管探查114例，结石阳性率53．5％；单纯胆囊切除311例，术后胆管残余结石3例。结论：胆囊切除术中胆管造影在减少胆总管阴性探查、降低胆管残石率、防止胆管损伤和提高胆石症治愈率等方面有较大临床应用价值。

Clinical application and evaluation of intraoperative cholangiography

Objective To evaluate the clinical value of intraoperative cholangiography.Methods From Oct.1993 to Oct.2001,737 cases of cholecystectomy divided into three groups were analyzed retrospectively.Results In 312 cases of cholangiography groups,135 cases were explorated in the common duct according to the intraoperative cholangiography,and the positive rate of choledocholith was 94 1%.In 114 cases of direct exploration groups the positive rate of choledocholith was 53 5%.In 311 cases of simple cholecystectomy groups,there were three postoperative residual calculus in biliary tract.Conclusion Intraoperative cholangiography can avoid unnecessary exploration in the common duct,reduce the residual calculus in the biliary tract,decrease the injury of biliary tract and increase the curative rate of cholelithiasis,which are of great practical value in clinic.
