双次自体外周血干细胞移植治疗NK/T细胞淋巴瘤再获缓解的研究

目的 探索双次自体外周血干细胞移植治疗NK/T细胞淋巴瘤的疗效.方法 对我科收治的1例NK/T细胞淋巴瘤进行双次自体外周血干细胞移植治疗的疗效及不良反应进行临床研究.结果 患者经EOAD Lasp 和EOADL MTX(2.0 g)方案化疗达完全缓解后行自体外周血干细胞移植,预处理方案:CEAC;移植 15d造血重建出层流病房,在造血稳定时采用大剂量IL-2行生物治疗;治疗过程中无严重不良反应发生.移植3个月后出现淋巴瘤细胞骨髓浸润,经VADD Lasp方案化疗无效的情况下再次行自体外周血干细胞移植,预处理方案:MOED MTX(3.0 g);移植 16d造血重建出层流病房;治疗过程中无严重不良反应及造血延迟发生;移植后骨髓完全缓解.患者已存活12个月.结论 双次自体外周血干细胞移植治疗NK/T细胞淋巴瘤的疗效肯定.

Study on second remission of NK/T lymphoma treated with double autologous peripheral blood stem cell transplantation

Objective To investigate the effect of NK/T lymphoma patient with double autologous peripheral blood stem cell transplantation(Auto-PBSCT).Methods To observe and analyse the therapeutic effect and adverse reaction of Auto-PBSCT for a patient with NK/T lymphoma.Results The patient reached complete remission after treated by EOAD Lasp and EOADL MTX(2.0 g).Then Auto-PBSCT was carried out after two cycles chemotherapy with EOADL MTX.The proposal for pre-disposal treatment was CEAC.The regeneration was achieved on 15d.After 2 weeks of regeneration,the patient received biotherapy with high dose IL-2(100wu every day for 5d of every week) for 28d.After three months of transplantation,massive cells of lymphoma were detected in bone marrow.No any effect was acquired with combined chemotherapy of VADD Lasp,then Auto-PBSCT was carried out again.MOED MTX(3.0 g) as pre-disposal treatment was carried out.The reconstitution was complete without delay and the bone marrow was also complete remission after transplantation.The man lived for 12 months.Conclusion The effect for NK/T lymphoma treated with double Auto-PBSCT is affirmative.
