内蒙古包头市农村地区白内障患者接受手术意愿及其相关因素调查

目的：调查内蒙古包头市农村地区白内障患者接受手术治疗的意愿及相关障碍因素。方法2012年对包头市所属4个郊县的49个乡镇年龄在50岁以上人群共11367人进行免费眼病筛查；所筛查出来的白内障患者进行整群随机抽样调查。调查对象为其中的10个乡镇，在这些乡镇中共2106人接受了免费的眼病筛查，初步被诊断为白内障的患者共494人。对494例患者进行接受手术的意愿及其相关障碍因素问卷调查。结果当场表示不愿意接受手术的患者共440人，占总数的89．1％。通过结构式访谈和问卷调查对影响患者接受白内障手术的因素进行logistic分析，其中年龄、性别、经济因素、复明的意识及与白内障及治疗的相关知识水平、对白内障手术的态度、对医院的信任程度、是否接触成功案例都影响了患者是否愿意接受白内障手术治疗，其中经济因素是最大的障碍因素。结论在内蒙古包头市农村地区白内障患者接受手术的意愿非常低（54/494）；不愿意接受手术的最大的障碍因素是经济因素。如何提高此类患者的治疗依从度是我们国家防盲政策应该思考的一个重要问题。

Investigation on the willingness and barrier factors for rural patients to receive cataract surgeries in Baotou of Inner Mongolia

Objective To investigate the patients′willingness to receive cataract surgeries in Baotou rural area of Inner Mongolia .Methods In 2012 Baotou Eye Hospital conducted free rural eye disease screening among 11 367 people aged over 50 years in 49 towns of 1 suburb and 3 counties .Patients screened as cataracts were selected as study popula-tion by cluster random sampling .2 106 rural villagers aged over 50 years in 10 towns received free eye disease screening and among them 494 were screened as cataract , who received questionnaire survey on their willingness to receive cataract surgeries and the barrier factors contributing to their unwillingness to receive cataract surgeries .Results There were 440 patients who expressed unwillingness to receive cataract surgeries on the spot , accounting for 89.1%of total study popu-lation.Structured interview and questionnaire had been used to investigate the barrier factors contributing to the unwilling -ness to receive cataract surgeries.Economic consideration, age, gender, related knowledge on cataract, the treatment of cataract , and the effectiveness of cataract surgeries , awareness to restore sight , attitude towards cataract surgeries , the in-fluence of local medical institute , and success of model patients were all factors contributing to the behavior of receiving cataract surgeries .Among them economic factor was the most powerful factor .Conclusion The biggest barrier factor for patients in receiving cataract surgeries in Baotou rural areas of Inner Mongolia is economic consideration .Policy makers have to explore an effective way to solve the problem by motivating patients to receive treatment and at the same time guar -anteeing medical institutions to develop with sustainability .More educational activities should be conducted to improve the knowledge , attitude and awareness of patients regarding cataract , cataract surgeries and its effectiveness .
