门静脉高压性胃病与幽门螺旋杆菌

了解门静脉高压患者胃幽门螺旋杆菌（Ｈ．Ｐｙｌｏｒｉ）感染及意义。方法应用胃镜，胃粘膜组织学检查和ＨＰ培养，调查１３５例门静脉高压患者，对照组为４２例无肝病史的非溃疡性消化不良患者。结果门静脉高压组，伴充血性胃病者６３例，１７例检出ＨＰ感染（２７０）；无充血性胃病者７２例，１８例检出ＨＰ（２５０）。对照组４２例，ＨＰ感染２３例（５５０）。门静脉高压性胃病ＨＰ感染低于对照组，充血性胃病存在及程度与ＨＰ感染无关。结论不应视ＨＰ感染为门静脉高压性胃病的发病原因，对肝硬化无须常规作ＨＰ根除治疗。

Helicobacter pylon and congestive gastropathy

Objective This study evaluates the prevalence and significance of Helicobacter pylon(H.pylori infection in patients with portal hypertension. Methods 177 patients were selectted, Among whom,135 patients with portal hypertension and 42 noncirrhotic patients with nonulcerative dyspepsia as acontrol group. In all patients diagnostic upper endoscopy was performed and gastric biopsies were takenfor histological examination and diagnosis of H. .pylori infection. Besults of the portal hypertensivepateints, 63 patients had congestive gustropathy, 17(27%) of whom were positive for H.pylori infectionand 72 patients did not have gnstropathy, 18(25%) of whom were positive for H.pylori infection. In thecontrol group 23 of 42(55%) were positive for H. .pylori infection. H. pylori was found less frequently incongestive gastropathy patients than in the control group. We also found that the presence and seventy ofcongntive gnstropathy is independent of the H.pylori status. Conclusion The role of H.Pylori infection inthe pathogenesis of congestive gastropathy seems to be talkely and we suggest that there is no need forits routine eradication in cirrhotic patients.
