子宫动脉化疗栓塞治疗子宫切口妊娠的疗效研究

目的探讨子宫动脉化疗栓塞术在治疗Ⅱ型子宫切口妊娠的临床疗效。方法回顾性分析2008年1月～2013年6月本院收治的36例Ⅱ型子宫切口妊娠（csPl患者采用双侧子宫动脉化疗栓塞术（UACE）为主的综合治疗。通过监测血绒毛膜促性腺激素（HCG），多普勒超声监测子宫切口部位病灶情况。观察术中出血量，月经恢复判断转归。结果36例患者中，6例子宫动脉化疗栓塞术＋期待治疗成功，27例子宫动脉化疗栓塞术＋超声监测下清官术治疗，术中平均失血35ml；2例子宫动脉化疗栓塞术＋经腹子宫切口病灶清除及修补术。1例子宫动脉化疗栓塞术＋经腹子宫次全切除术。结论子宫动脉化疗栓塞治疗Ⅱ型CSP疗效好，损伤小，能有效减少出血并保留子宫。是治疗Ⅱ型CSP安全、有效的方法。

Curative effect of uterine artery embolization chemotherapy in uterine incision pregnancy

Objective To study the clinical efficacy of uterine artery embolization chemotherapy in the treatment of type II uterine incision pregnancy. Methods Retrospectively analyzed 36 cases of type II uterine incision pregnancy（CSP） treated by bilateral uterine artery chemoembolization （UACE） comprehensive treatment from January 2008 to June 2013 in our hospital. By monitoring the blood human chorionic gonadotropin（HCG）, doppler ultrasound monitoring of uterine incision site lesions to observe the intraoperative blood loss, menstrual recovery determine outcome. Results Out of 36 cases of patients, 6 cases of uterine artery chemoembolization ＋ expectant treatment, 27 cases of uterine artery chemoembolization ＋ ultrasonic monitoring for curettage treatment, average intraoperative blood loss was 35 ml; 2 cases of uterine artery chemoembolization uterus abdominal incision clear and repair.1 case of uterine artery chemoembolization ＋ abdominal hysterectomy times total resection. Conclusion Uterine artery embolization chemotherapy has a good curative effect in the treatment of CSP with little injury, can effectively reduce bleeding and keep the uterus. So it is a safe and effective method for type II CSP.
