MMP-14与EMMPRIN在口腔鳞状细胞癌中的表达及临床意义

目的：探讨基质金属蛋白酶-14(matrix metalloproteinase 14，MMP-14)和细胞外基质金属蛋白酶诱导因子（extracellular matrix metalloproteinase inducer，EMMPRIN）在口腔鳞状细胞癌（oral squamous cell carcinoma，OSCC）中的临床意义，并分析二者的相关性。方法选取2012年1月—2014年6月于我院口腔颌面外科诊治的69例OSCC患者作为OSCC组，同时选取43例口腔正常新鲜组织标本作为对照组。采用逆转录-聚合酶链反应技术检测组织中的MMP-14及EMMPRIN mRNA的表达含量。结果MMP-14 mRNA、EMMPRIN mRNA在OSCC组中的相对表达量明显高于对照组，差异具有统计学意义（t=8.198、9.624，P＜0.01）；OSCC组患者在年龄、性别因素中的MMP-14、EMMPRIN mRNA的表达差异不明显（P＞0.05），在伴有淋巴结转移的患者中均明显升高（t=2.909、2.156，P＜0.05）；且MMP-14、EMMPRIN mRNA在Ⅲ～Ⅳ期、中低分化的OSCC中表达均明显升高，差异具有统计学意义（t=3.376、2.667、3.027、2.399，P＜0.05）；MMP-14与EMMPRIN mRNA表达含量间相关系数为r=0.821，二者呈正相关性（P＜0.01）。结论OSCC组织中MMP-14和EMMPRIN mRNA的高表达状态均与肿瘤的病理分期、分化程度及淋巴结转移因素关系密切，二者的表达具有正相关性，提示MMP-14和EMMPRIN均参与了OSCC的浸润及转移过程，通过检测EMMPRIN及MMP-14 mRNA表达含量可能有助于OSCC的病变程度及预后转归判断。

The clinical relevance of MMP-14 and EMMPRIN expressing in oral squamous cell carcinoma

ObjectiveTo discuss the clinical relevance of MMP-14 and EMMPRIN expressing in oral squamous cell carcinoma(OSCC).MethodSixty-nine cases of patients with OSCC were chosen from January 2012 to June 2014 in our hospital and were divided into the OSCC group. At the same time, 43 cases patients with normal oral tissues were chosen as the control group. The expression levels of MMP-14 and EMMPRIN mRNA were detected by using RT-PCR.Result The expression of MMP-14 and EMMPRIN mRNA in OSCC group was significantly higher than that of the control group (t=8.198, 9.624,P<0.01). Concerning OSCC, there were no statistical differences in the expression of EMMPRIN and MMP-14 mRNA in age and gender (P>0.05), but they increased significantly when in lymph node metastasis,Ⅲ~Ⅳ stage and moderate or poor differentiation (t=3.376, 2.667, 3.027, 2.399,P<0.05); the correlation coefficient between the expression of mRNA MMP-14 and EMMPRIN was 0.821, which indicated positive correlation (P<0.01).ConclusionThe expression levels of MMP-14 and EMMPRIN mRNA in the OSCC have a close relationship with pathological stage, differentiation and lymph node metastasis, which shows the MMP-14 and EMMPRIN are involved in the invasion and metastasis of OSCC. It can be helpful to estimate the severity and the prognosis of the OSCC by detecting the expression of MMP-14 and EMMPRIN mRNA.
