高频超声在闭合性尺神经病变中的应用

摘    要:目的探讨高频超声对闭合性尺神经病变的临床应用价值。方法分别对27例单侧闭合性尺神经病变患者行患侧和健侧相同节段水平尺神经高频超声多谱勒扫描,观察其声像图并进行对照分析。结果正常外周神经纵切面超声表现为中等条索状,内有线性平行回声,横切面呈圆形,内有点状回声。每1种病变都具有相应的声像图特征。16例尺神经卡压性病变可见内部低回声改变,外膜回声增强;6例尺神经粘连可见柬状回声模糊不清;5例尺神经肿瘤病变,可清晰显示肿瘤与神经的关系。结论高频多谱勒超声对闭合性尺神经病变的诊断和术式选择有重要指导意义。

Abstract:Objective To assess the value of the high-frequency ultrasound for closed lesions of ulnar nerve. Methods 27 patients with closed unilateral ulnar lesion were selected, whose ulnar nerve sonographie images of both affected and unaffected sides at the same level were obtained. Results The normal ulnar nerve manifested tubular eehogenie structure with parallel linear echoes longitudinally, while it took on a round echogenic structure with punetiform echoes transversely. Every type of lesion possessed its unique echoic images features.In 16 patients with compressive lesions centrally hypoechoic and peripherally hyperechoic changes can be seen. Ill-delineated fascicular pattern indicated nerve adhesion in 6 patients. In 5 patients with nerve tumor definitive relations between nerve and tumor were demonstrated. Conclusion High-frequency ultrasound is a good means to diagnose the closed ulnar lesion and can help to direct treatment.
