鼻胆管冲洗对ERCP术后胆总管结石复发的影响

目的探讨胆总管结石行ERCP术后鼻胆管冲洗对结石复发的影响。方法选择198例胆总管结石ER．CP术后患者，分为两组，鼻胆管冲洗组术后进行鼻胆管冲洗，常规对照组按常规处理。随访2年．比较胆总管结石复发率和复发后2次ERCP取石成功率。结果鼻胆管冲洗组14例结石复发，复发率（14．1％）低于常规对照组（31．3％），差异有统计学意义（P〈0．05）；鼻胆管冲洗组术后胆总管结石复发后2次ERCP取石成功率（100．0％）高于常规对照组（67．7％），差异有统计学意义（P〈0．05）。结论胆总管结石ERCP术后行鼻胆管冲洗是必要的，有助于防止结石复发。值得临床推广。

Effect of biliary tube irrigation after ERCP on recurrence of common duct stones

Objective To evaluate the prognostic value of biliary tube irrigation after ERCP for recurrence of common duct stones. Methods A total of 198 patients with common duct stones who underwent ERCP operations were selected and divided randomly into biliary tube irrigation group and simple operative treatment group. After 2 years＇follow-up visit, the recurrence rate of common duct stones and achievement ratio of recrudescent calculus removed by ERCP next time were compared between two groups. Results The recurrence rate of common duct stones of biliary tube irrigation group （14 cases, 14.1% ） was obviously lower than simple operative treatment group （31.3%）（P 〈 0.05）. Meanwhile, the achievement ratio of recrudescent calculus removed by ERCP next time of biliary tube irrigation group （100.0%）was obviously higher than simple operative treatment group（67.7%）（P 〈 0.05）. Conclusion Biliary tube irrigation after ERCP is essential, it can reduce the recurrence of common duct stones.
