脊髓缺血再灌注损伤中血流量变化与组织病理学相关性研究

摘    要:目的探讨缺血再灌注脊髓损伤(SCIfR)中的脊髓血流量(SCBF)与组织病理学变化规律相关性。方法阻断兔腹主动脉法制备腰段脊髓IR模型,实验组根据SCI/R时间段分为假手术组、缺检查。结果在缺血30min组腰髓局部血灌注量迅速下降至基线值的-77.51%(P=2.01E-17),有明确病理学改变。再灌注初始,局部血流迅速增高并超过基线水平,随后逐渐降低,再灌注1h后局部血灌注基本恢复基线水平(3.7%P值=0.437899),以后血流量低于基线水平,出现缺血后延迟低灌注;直至再灌注4h(-23.5%,P值=1.34E-3),低灌注保持相对稳定,血流未见恢复,病理学改变明显进一步加重。结论研究表明SCI/R血流动力学变化与组织病理学改变程度具有时间依从性,组织延迟性低灌注损伤是SCIfR重要病理学特点,为脊髓继发性损伤提供了理论依据。

Correlation-changes regularity in the histopathology and its spinal cord blood flow (SCBF)after spinal cord ischemia/reperfusion(I/R)

Abstract:Objective The aim of this experimental study was to investigate the correlation of histopathology changes and spinal cord blood flow (SCBF)after spinal cord ischemia/ reperfnsion(I/R) injury. Methods Rabbits were divided into six groups:sham group(no I/R), ischemia 30 rain group and ischemia 30 min/reperfusion grous (reperfusion grous 4,8,12 and 24 h) respectively. The bdominal aorta side was obstructed and opened according to events of isebemia-referfnsion. Laser-Doppler flowmetry was employed to measure hemodynamie changes and the lumb cord samples were rapidly taken for the histopathological evaluation. Results The lumbar local SCBF decreased rapidly to 77.51% (p = 2.01E- 17), percentage changes of the mean baseline value after 30 mins of ischemial. The onset of reperfusion, the SCBF increased rapidly and exceeded the baseline level. And the local blood perfusion basically recovered baseline level after 1 h of perfusion(3.7 % of baseline, P = 0.437 899). After wards, the SCBF decreased gradually. Up to 4 h of reperfusion( - 23.5 % of baseline, P = 1.34E - 3), the SCBF did not return to the baseline level. Conclusion Damage of spinal cord tissue in the hypoperfusion phase is serious than its isehemia phase, There are time correlation in the degree of pathology changes and hypopexfusion. It is impart,ant pathological feature that Spinal cord. tissue have has been occured delayed and low-reperfusion injury. The results from this study provide theoretical certification for the secondary dysfunction after the spinal cord iscbemie injury.
